【摘要】 目的 分析腹腔镜手术应用于治疗妊娠期卵巢肿瘤患者的临床疗效。方法 128例妊娠期卵巢肿瘤患者， 根据手术方式分为开腹组和腹腔镜组， 各64例。开腹组行开腹手术， 腹腔镜组行腹腔镜手术。比较两组患者术中及术后各指标情况及临床治疗有效性。结果 腹腔镜组患者手术时间、术中出血量、术后尿管留置时间、术后完全恢复时间、术后下床活动时间、平均住院时间、并发症发生率均优于开腹组， 差异均有统计学意义（P<0.05）。结论 采用腹腔镜治疗妊娠期卵巢肿瘤有显著的临床效果， 术中并发症少， 是临床上治疗妊娠期卵巢肿瘤的首选方法。

Investigation of clinical effect by laparoscopic surgery for 64 patients with ovarian tumor in gestation period GAO Hui-hua. Shandong Linyi Lanshan District Family Planning Maternal and Health Care Service Center， Linyi 276005， China

【Abstract】 Objective To analyze clinical effect by laparoscopic surgery in the treatment of patients with ovarian tumor in gestation period. Methods A total of 128 patients with ovarian tumor in gestation period were divided by operation way into laparotomy group and laparoscope group， with 64 cases in each group. The laparotomy group received laparotomy， and the laparoscope group received laparoscopic surgery. Comparison was made on intraoperative and postoperative indexes and effectiveness in clinical treatment between the two groups. Results The laparoscope group had all better operation time， intraoperative bleeding volume， postoperative indwelling catheter time， postoperative complete recovery time， postoperative off-bed time， average hospital stay time， and incidence of complications than the laparotomy group. Their differences all had statistical significance （P<0.05）. Conclusion Implement of laparoscopic surgery shows precisely clinical effect in treating patients with ovarian tumor in gestation period， with few intraoperative complications. It is the preferred clinical method in treatment.

妊娠期合并卵巢肿瘤在临床医学上较为常见， 其多半是先有卵巢肿瘤， 然后才妊娠[1]。妊娠与卵巢肿瘤同时存在时通常是以良性肿瘤为主， 恶性肿瘤相对较少[2]。由于女性生活和饮食的不规律使得该病近几年的发病率有上升趋势， 在很大程度上影响了孕妇的妊娠和分娩结局。因此， 对妊娠期合并卵巢肿瘤进行及时有效的诊断和治疗对孕妇和胎儿的预后起着关键的作用。以往妊娠期卵巢肿瘤的治疗方法就是采用传统开腹， 但因开腹手术术后恢复情况不佳， 患者的依从性较差。医疗技术的快速发展， 临床上应用腹腔镜的频率也逐渐升高， 为了提高妊娠期卵巢肿瘤的临床治疗效果， 本文将分别对患者采用腹腔镜手术和开腹手术治疗， 观察患者的术中术后指标改善情况， 探讨腹腔镜技术治疗妊娠期卵巢肿瘤患者的临床有效性， 具体研究报告整理如下。
