摘 要 目的：分析沪东社区管理重点孕妇的高危因素的变化趋势，找出社区孕期保健工作的重点。方法：回顾性分析沪东社区2012年1月-2014年12月2 962人孕产期管理相关资料。结果：橙色预警中需要用药的甲状腺疾病呈逐年升高，BMI异常患者、高龄孕妇、外地户籍孕妇、瘢痕子宫外地孕妇比例较前升高。结论：对社区孕妇进行妊娠风险预警评估十分重要，社区医生应根据本社区高危因素和特点有重点地指导孕妇进行更好的孕期保健。

Analysis of the changes of the risk factors for the key pregnant women in the community from 2012 to 2014

ABSTRACT Objective： To analyze the changing trend of the risk factors for the community management of the key pregnant women in Hudong Community to find out the emphasis of the community pregnant health care work. Methods： The relevant information of the management of 2 962 women in the pregnant period in Hudong Community from Jan. 2012 to Dec. 2014 was retrospectively analyzed. Results： The rate of hypothyroidism that required the medicine treatment increased in the orange signal warning. The proportions of the patients with the abnormal body mass index， women with the advanced maternal age， migrant women and the migrant women with scarred uterus increased more than before. Conclusion： It is important to make the pregnant risk assessment for the pregnant women in the community. According to the high risk factors and characteristics of this community， the community doctors should focus on the guidance of the pregnant women for the better health care during the pregnancy.

对社区重点孕妇的管理是孕产期保健的重要内容之一。经研究证明，重点疾病筛查较已实行了20多年的高危孕妇管理模式更具优越性、筛查更科学、管理更严密、效果更显著[1]。现将我社区2012年1月至2014年12月社区重点孕妇筛查结果进行分析。

1 资料与方法

1.1 研究对象和调查方法

资料来源于上海市沪东社区2012年1月-2014年12月孕产妇系统管理登记本，重点孕妇登记本以及孕产妇系统管理报表进行分析，3年中社区住院分娩2 962人，本市户籍1 506人，非本市户籍1 456人。研究人群无妊娠次数限制。

1.2 妊娠风险评估方法

每例孕妇早孕期在本社区卫生服务中心建册时，依据由上海市妇女保健所印制的《妊娠风险预警评估表（社区使用）》进行第1次筛查，对筛查阳性、疑似重点疾病者填写“风险预警转诊单”，由孕妇产检时带至二级及以上医疗机构进行确诊，并将风险预警转诊单填写完整后汇总至浦东新区妇幼保健所。……
