摘 要 目的：确保手术室特殊药品管理严格而规范。方法：设立麻醉药箱，实行基数管理；特殊药品处方电子化；加强特殊药品剩余药液管理。结果与结论：杜绝手术室特殊药品非法盗用和流失，节省药师和麻醉师工时，推进特殊药品合理利用的监测。

Difficulties in and countermeasure to the standardizing management of special drugs at operation pharmacy

ABSTRACT Objective： To ensure the strict and normative management of special drugs in operation rooms. Methods： Narcotic medicine chest was established to implement cardinal number management and the prescriptions of special drugs were electronized and the management of residual liquid of special drugs was strengthened. Results & Conclusion： The illegal embezzlement and loss of special drugs in operation rooms can be completely eradicated and the working hours of pharmacists and anesthesiologists can be saved so as to promote the monitoring of the rational use of special drugs.

特殊药品是指麻醉药品、精神药品、易制毒化学品和医疗用毒性药品及参照麻醉药品管理的药品[1]。手术室和麻醉科是我院特殊药品使用最为集中和使用量最大的科室，对手术室特殊药品进行严格而规范的管理，一直是落实医疗机构特殊药品管理制度的重中之重。我院手术药房对特殊药品管理工作中的一些难点进行梳理，并提出相应对策。

1 特殊药品基数管理

手术室特殊药品申领、调剂和使用流程不同于门急诊药房和住院药房，属于“先用药、后结算”的动态模式。手术药房执行医疗机构麻醉药品“五专”管理制度、《麻醉药品管理办法》、《精神药品管理办法》、《易制毒化学品管理条例》、《医疗用毒性药品管理办法》的难度不大，但监督手术室特殊药品相关管理制度的落实难度却不小，应在保障临床用药方便快捷和严格执行特殊药品管理法规之间寻找平衡点。

手术药房根据不同手术的需求和麻醉师用药习惯，设立6种麻醉药箱，实行基数管理。6种麻醉药箱分别为：常规麻醉药箱、急诊麻醉药箱、心血管麻醉药箱、内镜麻醉药箱、周末值班麻醉药箱和移植麻醉药箱。……
