老年髋部骨折行内固定术与人工髋关节置换术的临床对比研究

【摘要】目的 比较老年髋部骨折行内固定术与人工髋关节置换术的临床疗效。方法 将我院2013年1月～2014年2月收治的60例老年髋部骨折患者按抽签方式分为2组，均30例。对照组接受内固定术治疗，观察组接受人工髋关节置换术治疗。比较2组临床治疗效果。结果 对照组髋关节功能优良率和术中出血量低于观察组，差异有统计学意义（P＜0.05）；2组的术后引流量相比，差异无统计学意义（P＞0.05）。结论 对老年髋部骨折实施人工髋关节置换术治疗，可有效促进髋关节功能的恢复，有助于改善预后效果。

Objective To compare the clinical effect of internal fixation and hip replacement in elderly patients with hip fracture. Methods 60 cases of hip fracture in elderly patients in our hospital from January 2013 to February 2014 were divided into 2 groups by the drawing of lots，30 cases each. The control group received internal fixation treatment，observation group received total hip arthroplasty. Two groups were compared of clinical outcomes. Results The control group of good rate about hip joint function and blood loss was significantly lower than the observation group，the difference was significant （P＜0.05）. 2 groupsof postoperative drainage compared had no significant difference（P＞0.05）. Conclusion Elderly hip fracture for artificial hip replacement therapy can effectively promote the recovery of hip function，help to improve the prognosis.

髋部骨折是临床常见的一种骨折，多发于老年人群［1］。髋部骨折患者常表现出髋关节局部疼痛和活动受限等症状，严重影响患者的生活和工作［2］。因此如何有效治疗老年髋部骨折，是临床医师面临的棘手问题［3］。本研究对60例老年髋部骨折患分组实施内固定术与人工髋关节置换术治疗，对比临床治疗效果。报道如下。

1　资料与方法

1.1一般资料

将我院2013年1月～2014年2月收治的60例老年髋部骨折患者按抽签方式分为2组，均30例。对照组男17例，女13例；年龄60～79岁；平均年龄（65.47±3.63）岁。观察组男16例，女14例；年龄61～81岁；平均年龄（65.24±3.87）岁。2组的一般资料相比，差异无统计学意义（P＞0.05），可进行对比。

对照组接受内固定术治疗：进行常规麻醉，取仰卧位，患侧髋粗隆部垫高，沿135°导向器插入……
