无症状性心肌缺血患者的动态心电图诊断研究

【摘要】目的探究无症状性心肌缺血患者动态心电图的诊断价值。方法选取2014年10月～2015年10月收治的89例心肌缺血患者进行动态心电图的诊断，实验组为45例无症状性心肌缺血患者，对照组为44例有症状的心肌缺血患者。结果两组患者在ST段的降低幅度差异无统计学意义（P＞0.05）。实验组患者的心肌缺血发作持续时间大于对照组（P＜0.05）。且实验组患者心肌缺血的检出率和发作次数高于对照组（P＜0.05）。结论无症状性心肌缺血患者采用动态心电图诊断，可有效检查出患者的发作情况，为治疗提供有效依据。

Objective Dynamic ECG（electrocardiogram）in diagnosis of patients with silent myocardial ischemia is to be investigated.Methods Chose 89 patients with silent myocardial ischemia who were treated in hospital from October 2014 to October 2015 to diagnose with dynamic ECG，45 patients selected in study group with silent myocardial ischemia，while another 44 patients selected in control group with symptomatic myocardial ischemia.Results There was no differential between patients in two groups in terms of ST segment reduction degree（P＞0.05）.But patients'myocardial ischemia attack during time in study group was longer than that in control group（P＜0.05），and detection rate and attack times for myocardial ischemia were greatly higher than counterparts in control group（P＜0.05）.Conclusion Dynamic ECG（electrocardiogram）is quite effective in diagnosis of patients with silent myocardial ischemia.It is conducive to diagnosing patients'illness attack，which is important to provide accurate evidence for clinical treatment.

心电图的检查有无创、快捷方便等优点，为重要的临床诊断和监测方法，和常规心电图相比，动态心电图可在24 h内连续记录10万次左右心电信号，动态的心电图可长时间记录分析心脏在安静和活动状态下的心电图变化［1］。无症状的心肌缺血属于隐形冠心病，这类患者本身没有明显的心绞痛，但是心肌血流灌注和心肌代谢以及左心室功能都有异常，还有冠状动脉的病变情况，但是因为患者没有不适症状，所以易忽视，普通的心电图检查，一般检查不出［2］。本次研究选取2014年10月～2015年10月收治的89例心肌缺血患者进行动态心电图的诊断，现报告如下。

1　资料和方法

1.1一般资料

本次选取2014年10月～2015年10月收治的89例心肌缺血患者进行动态心电图的诊断，实验组患者45例，男性25例，女性20例，最小年龄44岁，最大年龄87岁，平均年龄为（57.42±8.13）岁。……
