激素依赖性皮炎的诊断与外治疗法

摘要：目的分析激素依赖性皮炎的诊断与鉴别方法，并对比外治疗法疗效。方法选取激素依赖性皮炎85例作为研究对象，随机分为治疗组45例和对照组40例，对照组单独使用吡美莫司软膏治疗，治疗组给予吡美莫司软膏联合重组人表皮生长因子凝胶进行治疗，观察并对比2组临床效果、复发以及不良反应发生情况。结果治疗组总有效率97.8%，复发率为8.9%，无任何不良反应发生；对照组总有效率77.5%，复发率37.5%，不良反应的发生率为7.5%；2组总有效率和复发率比较，差异有统计学意义(P<0.05)，不良反应发生率比较，P>0.05。治疗组1、2个月内愈合率均高于对照组，P<0.05。结论吡美莫司软膏联合重组人表皮生长因子凝胶治疗激素依赖性皮炎，总有效率高，复发率低，且不易复发。

Abstract：ObjectiveTo analyze the diagnosis and treatment of hormone-dependent dermatitis and compare the clinical effect between single tacrolimus ointment and tacrolimus ointment combined with recombinant human epidermal growth factor gel, in order to provide certain theoretical basis for the clinical treatment of hormone-dependent dermatitis. MethodsThe 85 cases with hormone-dependent dermatitis were selected as the research object and divided into the treatment group (45) and the control group (40) randomly. The control group was given the single tacrolimus ointment treatment and the treatment group was given the tacrolimus ointment combined with recombinant human epidermal growth factor gel treatment. Clinical effect, relapse and adverse reactions occur of the two groups were compared and observed. ResultsThe clinical total effective rate of the experimental group was 97.8% and the recurrence rate was 8.9%, without any adverse reactions occur; the clinical total effective rate of the control group was 77.5%, the recurrence rate was 37.5%, and the incidence of adverse reactions was 7.5%; the clinical total effective rate and recurrence rate of the two groups had significant differences, the difference was significant statistically significant (P<0.05). The incidence of adverse reactions was no significant difference (P> 0.05). Healing rate within 1 month and 2 months of the treatment group were higher than that of the control group (P< 0.05). Conclusion The tacrolimus ointment combined with recombinant human epidermal growth factor gel on the treatment of hormone-dependent dermatitis has remarkable curative effect, which is not easy to relapse and has certain clinical application value.

目前国内对于激素依赖性皮炎的诊断尚无统一标准，国外对于激素依赖性皮炎的诊断主要依据明确的外用激素史、典型的类酒渣鼻样皮损以及激素停用后皮损反复等[1-2]。根据以上诊断依据并不能指导医生认识原发面部疾病的复发和激素依赖性皮炎反跳现象之间的区别，容易误诊[3-4]。因此总结激素依赖性皮炎的临床特点及制定新的诊断依据是十分必要的，本院选取部分激素依赖性皮炎患者进行研究，通过查阅大量的国内外相关文献报道，总结激素依赖性皮炎特有的临床表现、诊断切入点以及临床治疗等。……
