检测H型高血压患者血清Cysc与β2-MG对早期肾损害的临床意义*

【摘要】目的探讨H型高血压患者血清半胱氨酸蛋白酶抑制剂C(Cysc)与β2微球蛋白(β2-MG))检测对此类患者早期肾功能损害的临床意义。 方法选择2013年7月～2014年7月收治住院的高血压患者104例，分别测定血清同型半胱氨酸(Hcy)、半胱氨酸蛋白酶抑制剂C及β2-微球蛋白。将血清Hcy≥15μmol/L分为H型高血压组39例和Hcy<15μmol/L为非H型高血压组65例，比较两组患者检测结果的差异性。 结果H型高血压组与非H型高血压组血清Cysc、β2-MG比较，差异均有统计学意义(均P<0.01)。 结论血清Cysc、β2-MG对H型高血压患者早期肾功能损害的诊断及治疗具有重要价值。

Abstract【】ObjectiveTo explore the clinic diagnoses value of H type hypertension in early renal damage throungh detection of serum β2-microglobulin (β2-MG) and Cystatin (Cysc).MethodsThe serum Homocysteine (Hcy)and Cysc and β2-MG in 104 hospitalization hypertension patients were detected. The serum Hcy≥15μmol/L were divided into the groups of H type hypertension(39 cases). The serum Hcy<15μmol/L were divided into the group of non H type hypertension(65 cases), in order to compare with the differences between two groups detected. Results The serum β2-MG and Cysc in the group of H type hypertension compared with the group of non-H type hypertension. The difference between two groups were statistical significance (P<0.01). ConclusionThe detection of serum Cysc and β2-MG of H type hypertension has important value and significant in discovery and diagnose in early renal damage.

随着我国高血压发病率逐年增加，由高血压引起的严重心、脑血管疾病成为慢性病的主要死亡原因。在我国，H型高血压在原发性高血压患者中与欧美国家相比显著居多，因此，H型高血压是我国诱发各种心脑血管疾病的元凶[1]。但其对肾脏的损害常常因为随着患病时间的延长才缓慢出现而被忽略。故早期诊断H型高血压所致的肾损害，对防止或延缓末期肾病、降低高血压死亡率和改善患者生活质量具有重要意义。高Hcy标准各家不一，我院采用Hcy≥15μmol/L归为高Hcy血症，把伴有高Hcy血症的原发性高血压归为H型高血压。本文通过对H型高血压与非H型高血压血清Cysc、β2-MG检测比较，了解H型高血压早期肾功能损害的情况，进一步实施有效的干预，以减少肾损害的发生。

1资料与方法

1.1临床资料收集我院2013年7月～2014年7月住院的高血压患者104例，男41例(39.4%)，女63例(60.6%)，年龄21～88岁，平均(64.57±13.48)岁，均符合2010年中国高血压防治指南[2]诊断标准。……
