【摘要】目的 探讨超声对于医源性外周血管损伤的诊断和治疗价值。方法 回顾性分析34例医源性外周血管损伤的声像图特点以及超声在其治疗中起到的作用。结果 超声定性诊断医源性外周血管损伤的准确率为100%，定位治疗的成功率为90%。结论 超声对于医源性外周血管损伤的定性诊断、定位治疗和预后评价有重要的临床应用价值。

Diagnostic and therapeutic values of ultrasound

in iatrogenic peripheral vessels injury

【Abstract】Objective To explore the value of color Doppler ultrasound in diagnosis and therapy of iatrogenic peripheral vessels injury. Method Thirty-four patients with iatrogenic peripheral vessels injury in the Methods of ultrasonogram characters and therapeutic effectiveness by ultrasound were retrospective analysised.Results The accuracy rate of qualitative diagnosis by ultrasound was 100%， and the achievement ratio of localization therapy by ultrasound was 90%. Conclusion Ultrasound have great clinical applied values in diagnosis and therapy and prognostic evaluation of iatrogenic peripheral vessels injury.

外周动静脉穿刺为心脏介入诊疗常见手术入径。本文回顾性分析本院彩色多普勒超声参与诊治的34例上下肢动静脉医源性损伤，探讨其临床应用的价值。

1 资料与方法

1.1 一般资料

2010年1月～2013年6月在本院接受冠状动脉造影术29例，先心病介入治疗手术患者5例，共34例；男21例，女13例，年龄7～65岁，平均年龄41岁。

1.2 方法

根据血管损伤部位深浅不同选择腹部凸阵探头或线阵高频探头。患者仰卧位，暴露患侧肢体，于穿刺点附近扫查。二维超声观察穿刺部位肿块的数目、形态，测量大小，显示肿块与周围动静脉关系、有无破口或瘘道，做出定性诊断；测量破口或瘘道长短及最大内径，彩色多普勒观察显示肿块血流情况，脉冲多普勒测量通道口血流参数。

2 结 果

本院共行诊疗性股动静脉和桡动脉穿刺2561人次。术后经本院超声诊断医源性外周血管损伤34例，检出率约1.33%。经临床治疗及随访证实符合率100%。其中穿刺部位血肿（HMA）14例，深静脉血栓（DVT）形成5例，假性动脉瘤（PSA）12例，动静脉瘘（AVF）4例，见表1。……
