【摘要】 目的：分析腹腔镜在腹部闭合性损伤中的应用价值。方法：回顾59例腹部闭合伤患者的临床资料及诊断、治疗效果。结果：探查59例均明确诊断损伤脏器、部位。其中3例未作特殊处理，23例在镜下或腹腔镜辅助下得到治疗，33例中转开腹手术。6例术后有并发症，无死亡病例。全组均治愈出院。结论：腹腔镜探查术应用于腹部闭合伤，可达到精确诊断、镜下治疗、降低阴性剖腹探查等目的，并能指明中转开腹方向。

【Abstract】 Objective：To investigate the application value of laparoscope in the treatment of closed abdominal trauma.Method：Clinical date，diagnosis and treatment outcome of 59 patients with closed abdominal trauma were retrospectively analyzed.Result：Organs and site were examined for the 59 patients with confirmed closed abdominal trauma.Of all cases，3 cases didnt received any particular treatment，23 cases received treatment with the assistance of endoscopy or laparoscope，33 cases received laparotomy and 6 cases presented infection after operation.No death cases were present and all cases were successfully cured.Conclusion：The application of laparoscope during treatment of closed abdominal trauma provides precise diagnosis and treatment，reduces the necessity of laparotomy，as well as indicates the orientation during laparotomy.

以工业意外和交通事故为主要原因的腹部闭合伤，占全部创伤的1.5%～18%[1]。临床表现复杂多样，且大约1/3病例早期可无典型临床表现，早期诊断是否存在脏器损伤或何处脏器损伤有时十分困难。而腹腔镜探查安全、准确、全面的特点，为腹部闭合伤诊治提供了很好的途徑。笔者所在医院2008年1月-2012年12月，共收治腹部闭合伤81例，其中59例应用腹腔镜进行探查和治疗，取得较满意效果，现总结报告如下。

1 资料与方法

1.1 一般资料

本组59例中男48例，女11例，年龄16～63岁。致伤原因：车祸致伤24例，高处坠落10例，钝器撞击伤16例，挤压伤9例。59例均有不同程度腹痛、腹胀、呕吐，其中47例有腹膜刺激征。血流动力学基本稳定，或经积极补液复苏后基本稳定。所有病例术前经超声、X线、腹穿等检查，诊断或疑似内脏损伤。

1.2 方法

回顾59例腹部闭合伤患者的临床资料及诊断、治疗效果。所有病例均于全麻下行腹腔镜探查术。按急诊开腹术前准备，术前疑诊肝、脾损伤的垫好腰垫。……
