【摘要】 目的：观察研究鼻内镜下行鼻腔、鼻窦内翻性乳头状瘤切除术的治疗效果和可行性。方法：对在笔者所在医院行鼻内镜下鼻腔、鼻窦内翻性乳头状瘤切除术的34例患者的术前评估、治疗过程和术后随访资料进行回顾性分析。结果：其中17例I级、15例Ⅱ级病例行鼻内镜下鼻腔、鼻窦内翻性乳头状瘤切除术，2例Ⅲ级患者行鼻内镜加Caldwell-Luc术。术后随访2～6年，治疗效果良好，复发率低，4例复发，无一例患者发生恶变。结论：鼻内镜下手术是治疗Krouse I～Ⅱ级鼻腔、鼻窦内翻性乳头状瘤的一种可行的方法。鼻内镜下手术优点：手术视野清楚，创伤小，出血较少，低复发率，无面部切口及瘢痕。

【Abstract】 Objective：To evaluate the curative effect of endoscopic surgery（ESS） in the treatment of nasal cavity and sinuses inverted papilloma.Method：We retrospectively analyzed 34 cases of endoscopic nasal cavity and sinuses inverted papilloma resection in patients with preoperative patient assessment，treatment and postoperative follow-up data.Result：17 cases with grade Ⅰ and 15 cases with grade Ⅱ patients of nasal cavity and sinuses inverted papilloma treated with endoscopic surgery，2 cases of grade Ⅲ patients with nasal endoscopy and Caldwell-Luc operation.The operation effect was good，the follow-up 2-6 years，4 cases of recurrence，and all the patients without malignant transformation.Conclusion：The nasal endoscopy is a better method for treating Krouse Ⅰ-Ⅱ stage nasal inverted papilloma.The advantages of endoscopic surgery include clear visual field，less bleeding，limited reoccurrence，and avoidance of facial incision.

鼻腔、鼻竇内翻性乳头状瘤（nasal cavity and sinuses inverted papilloma，NIP）是耳鼻咽喉科常见的良性肿瘤，多发于中老年男性，占鼻部肿瘤的0.5%～.0%[1]，具有高侵袭性、高复发率（6.0%～8.0%）、恶变倾向（7.0%～7.0%）的临床特点[2]，手术能否彻底切除病变组织是治疗该病成功与否的关键。传统的手术方法多采用鼻侧切开径路，手术损伤大，手术后面部遗留下瘢痕，影响美观，致使多数患者难以接受。随着鼻内镜手术技术的日益成熟，鼻内镜下治疗鼻腔、鼻窦内翻性乳头状瘤的优点逐渐凸显，其优点是在彻底切除肿瘤组织的同时能最大限度地保留鼻腔、鼻窦正常的黏膜和结构，组织创伤小，疗效更好。2007年1月-2011年12月，笔者所在科室34例经鼻内镜下切除鼻腔、鼻窦内翻性乳头状瘤患者，经2～6年随访，治疗效果满意。……
