【摘要】 目的：探讨和分析病案管理中易发生医疗纠纷的环节和预防措施。方法：加强对病案质量的控制和对病案环节的管理。结果：2012年医疗纠纷发生率2.55%（51/2000）明显低于2011年的7.50%（150/2000），法律知识掌握人数1458例明显多于2011年的560例，患者的满意度94.75%（1895/2000）明显高于2011年的60.0%（1200/2000），比较差异均有统计学意义（P<0.05）。结论：在逐步实行病案双轨制管理和医疗纠纷逐渐增多的情况下，要增强医院的法律意识，同时加强对病案的管理，能够有效地减少和避免医疗纠纷的发生。

【Abstract】 Objective：To investigate and analyze the medical record management in medical dispute link and preventive measures.Method：Strengthen the control of medical record quality and the management of the medical record link.Result：Incidence of medical disputes in 2012 was 2.55% （51/2000）， it was significantly lower than the 7.50% in 2011 （150/2000）， the number of legal knowledge in 2012 was 1458 cases，it was more than 560 cases in 2011， the satisfaction rate of patients was 94.75% （1895/2000） ，it was significantly higher than the 60.0% in 2011 （1200/2000）， the differences had statistical significance（P<0.05）.Conclusion：Medical record management and medical disputes are applicable shall be gradually practised in the gradually increasing number of cases，to enhance the legal awareness of hospital，strengthen the management of medical record at the same time，can effectively reduce and avoid the occurrence of medical disputes.

随着社会的不断发展和进步，信息技术也在飞速发展，卫生信息化程度在逐渐提高，在很多医院都实现电子病案系统来采集病案资料[1]。但是，目前电子病案还不具备相关的法律效力，因此，很多医院实行纸质病案和电子病案同行的双轨制病案管理。在对纸质病案进行保管时，同时对电子病案进行收集和管理[2]。然而，在这样双轨制的管理下，增加了工作人员的工作难度和强度，同时也给医院医疗安全带来很大的隐患。此外，随着我国法律的不断完善以及人们维权意识不断增强，导致医疗纠纷事件不断增加。对于如何减少和控制医疗纠纷事件的发生，本院从病案管理方面进行着手，目前取得了一定的成效，现报告如下。

1 资料与方法

1.1 一般资料 回顾性分析总结2011-2012年本院病案管理的相关资料，此次研究对象有2000例患者，其中男1200例，女800例，年龄21～60岁，平均（36.0±2.5）岁。

1.2 调查方式 通过查阅资料的方式对每年出现的医疗纠纷事件进行登记，同时分析发生的原因。狭义的医疗纠纷就是指医疗机构在为患有某种疾病、伤痛或功能障碍的患者提供单纯的医疗康复服务或者病案记录过程中，出现了不良后果或者病案信息错误，由此产生异议而引发的争议。……
