[摘要] 目的 探讨骨盆骨折并发失血性休克的急诊救治方法。 方法 回顾分析笔者所在医院急诊外科2009年6月～2012年5月收治的32例骨盆骨折并发失血性休克患者的临床资料，并将患者的救治结果与2004年6月～2009年5月间收治的60例该类患者的相关资料进行比较，探讨骨盆骨折并发失血性休克的急诊救治方法。 结果 在急诊综合救治模式下，32例患者中24例在入院后1 h内纠正休克，7例在入院后6 h内纠正休克，1例死亡。与过去5年间该类患者的数据比较，救治成功率高。 结论 急诊绿色通道、急诊病房、急诊手术、急诊重症监护室的综合救治模式能快速纠正骨盆骨折并发失血性休克患者的休克状态，明显提高患者的存活率，值得临床推广。

Emergency experience of treating pelvic fracture with hemorrhagic shock

[Abstract] Objective To investigate the emergency treatment for the patients with pelvic fracture complicated with hemorrhagic shock. Methods Retrospective analysis the data of 32 cases of pelvic fracture complicated with hemorrhagic shock patients tobe treated in the emergency surgery in the author's hospital from June 2009 to May 2012,and associated comparison with the data of 60 patients to be treated from June 2004 to May 2009,investigating the emergency treatment for the patients with pelvic fractures complicated with hemorrhagic shock. Results Through the comprehensive treatment,24 patients correct shock within 1 hour after hospitalized,7 patients correct shock within 6 hours,1 patient dead. Comparing the date of past 5 years, the success rate of treatment is higher. Conclusion Under the comprehensive treatment mode include emergency easy access,emergency wards,emergency operation and emergency ICU can quickly correct the shock state of pelvic fracture complicated with hemorrhagic shock,improve the patient survival rate,worthy of clinical application.

随着社会经济的飞速发展，交通事故及高处坠落伤等日益增多。Eastridge等[1]统计，在复合外伤中，骨盆骨折并发失血性休克所占的比例达3%～5%，此类疾病病情凶险，死亡率高。如何在黄金1 h内使患者脱离休克状态，为后期治疗打下基础，是广大临床一线的医务工作者面临的难题[2]。笔者所在科室在结合急诊绿色通道、急诊手术、急诊病房及急诊重症监护室的综合模式下对该类患者进行救治，取得了满意的效果，现报道如下。

1?资料与方法

本组病例资料取自2009年6月～2012年5月来笔者所在科室就诊并确诊为骨盆骨折并发失血性休克患者32例。……
