[摘要] 目的 规范管理濮阳地区基层医疗机构临床输血，控制输血风险，安全有效输血。 方法 调查分析基层医疗机构临床输血存在的诸多问题和存在的风险，采取培训指导、建章立制、合格准入、综合考评、动态管理等有效措施。 结果 基层医疗机构临床用血风险得到有效控制，消除了潜在的问题。 结论 通过对基层医疗机构临床输血专项管理和标准化血库建设，规范了临床用血行为，确保了用血安全。

[Abstract] Objective Standardized management of primary medical institutions in Puyang area of clinical blood transfusion, control of transfusion risk, effective and safe transfusion. MethodsInvestigation and analysis of basic medical institutions in clinical blood transfusion existence many problems and risks, take training guidance, build a chapter set system, qualified admittance, comprehensive evaluation, dynamic management and other effective measures. Results Primary medical institutions clinical blood use to control risks effectively, eliminate the potential risk. Conclusion Through the primary medical institutions clinical blood transfusion management and standardization of blood bank construction, normative clinical blood use behavior, ensure the safety of blood transfusion.

输血实际上是一种类似器官移植的干预，存在着发生输血不良反应和输血相关疾病传播的风险。近几年我市对所辖五县二区的98家二级以下医疗机构、乡镇卫生院、社区服务中心等基层医疗机构临床用血实行了专项管理，开展标准化血库创建活动。依据卫生部《医疗机构临床用血管理办法（试行）》[1]和《临床输血技术规范》[2]，结合本地区实际制定了《濮阳市乡镇卫生院临床输血管理及血库标准化建设考评细则》（以下简称《细则》），城市二级以下医疗机构开展临床输血工作要符合《河南省医疗机构血库基本标准》[3]要求。各用血医疗机构着力加强临床用血管理和血库标准化建设，以病人为中心，提高输血质量，降低输血风险，取得了显著成效。现分析如下。

1基本情况

1.1医疗机构

医疗机构数量多（98家）、规模小（平均床位30张）、用血量少（2011年用血量红细胞500u）,在数量上占全市临床用血医院的90%（98/110），但用血量只占全市用血总量的1.01%（500/49500）。……
