摘要：目的：研究卡格列凈对2型糖尿病合并冠心病患者心血管危险因素及心功能的影响。方法：选择2020年8月至2021年6月在惠州市第三人民医院就诊的2型糖尿病合并冠心病患者60例，采用随机数字法分为对照组（30例）、观察组（30例），对照组予二甲双胍加格列齐特治疗，观察组予二甲双胍加卡格列净治疗，治疗12周，比较两组治疗前后血压（BP），空腹血糖（FPG）、餐后2小时血糖（2hPBG）、血脂、糖化血红蛋白（HbA1c）、高敏肌钙蛋白I（hs-cTnI）、B型脑钠肽（BNP）、左室射血分数（LVEF）、左室舒张末期内径（LVEDD）、舒张早期最大峰值速度（E峰）、舒张晚期最大峰值速度（A峰）、E/A的变化。结果：两组治疗后FPG、2hPBG、HbA1c均比治疗前下降（P<0.05），两组治疗后FPG、2hPBG、HbA1c、舒张压、hs-cTnI组间差异无统计学意义;治疗后，观察组甘油三酯（TG）、总胆固醇（TC）、低密度脂蛋白胆固醇（LDL-C）、收缩压、NT-proBNP、LVEDD、A峰均比对照组降低，高密度脂蛋白胆固醇（HDL-C）、LVEF、E峰、E/A 均比对照组增高（P<0.05）。结论：卡格列净联合二甲双胍可降低2型糖尿病合并冠心病患者血糖、血脂水平，改善心功能，不增加低血糖发生率，安全性较高。

[Abstract]Objective：To investigate the influence of canagliflozin on cardiovascular risk factors and heart function in patients with type 2 diabetes mellitus（T2DM）complicated by coronary heart disease（CHD）.Methods：The patients with T2DM complicated by CHD（n=60）were chosen from the Third People's Hospital of Huizhou City from August 2020 to June 2021，and then divided into control group（n=30） and observation group（n=30） according to random digital table.The control group was orally given metformin tablets and gliclazide，the observation group was orally given metformin tablets and canagliflozin for 12 weeks of treatment.The levels of blood pressure，fasting plasma glucose（FPG），2-hour postprandial blood glucose（2hPBG），blood fat，glycated hemoglobin（HbA1c），High-sensitivity troponin I（hs-cTnI），B-type brain natriuretic peptide（BNP），left ventricular ejection fraction（LVEF），left ventricular end-diastolic inner diameter（LVEDD），early diastole inflow velocity（E），late diastole inflow velocity（A），early diastole inflow velocity/late diastole inflow velocity（E/A），were compared between 2 groups.Results：After treatment，the levels of FPG、2hPBG、HbA1c were reduced than before measured in 2 groups（P<0.05），but there were no significant differences of FPG、2hPBG、HbA1c、diastolic blood pressure、hs-cTnI between 2 groups after treatment.After treatment，the levels of triglycerides （TG）、total cholesterol（TC）、low density lipoprotein-cholesterol（LDL-C）、systolic blood pressure、NT-proBNP、LVEDD and A were lower，and levels of high density lipoprotein-cholesterol（HDL-C）、LVEF、E、E/A were higher in observation group than those in control group（P<0.05）.Conclusion：Canagliflozin combined with metformin can reduce levels of blood sugar and blood fat，and improve heart function in patients with type 2 diabetes mellitus（T2DM）complicated by coronary heart disease（CHD），and does not raise incidence of hypoglycemia with higher safety.
