摘要：目的 观察国医大师张震疏调通便方治疗阿片类药物所致便秘的临床疗效。方法 采用随机对照的设计，将66例患者随机分成治疗组33例（疏调通便方颗粒）和对照组33例（麻仁软胶囊），2组均治疗7 d后，观察比较2组患者治疗前后生活质量评分、便秘症状评分、便秘疗效、中医伴随症状情况。结果 治疗组便秘疗效总有效率为96.7%，对照组总有效率为62.5%，2组总有效率比较差异有统计学意义（P<0.05）;2组患者中医伴随症状积分治疗后较治疗前均有明显改善，治疗组改善情况明显优于对照组，差异均有统计学意义（P<0.05）。结论 国医大师张震疏调通便方颗粒治疗阿片类便秘有显著疗效，值得临床推广应用。

Clinical Observation on Traditional Chinese Medicine Master Zhang ZhensConstipation Relief Decoction for Opioid-induced Fecal Impaction

【Abstract】Objective： To observe the clinical effect of traditional Chinese medicine master Zhang Zhen's constipation relief decoction for opioid-induced fecal impaction. Methods： We designed a randomized controlled trial： 66 patients were randomly divided into the treatment group of 33 patients （Zhangzhen's Constipation Relief Granules） and the control group of 33 patients （Maren Soft Capsules）. The patients in both groups were treated for 7 days， and their life quality score， constipation symptom score， constipation efficacy and common accompanying symptoms by traditional Chinese medicine were evaluated and analyzed before and after treatment. Results： The total cure rate of constipation in the treatment group was 96.7% in contrast to 62.5% in the control group with statistically significant difference between the two groups （P<0.05）. In addition， accompanying symptom score of patients in the treatment group significantly improved compared with the control group （P<0.05）. Conclusion： Traditional Chinese Medicine Master Zhang Zhen's recipe has a significant effect on opioid-induced constipation， and it is promising for clinical application.

阿片類便秘（opioid-induced constipation，OIC）不仅出现于应用阿片类药物的初期，而且持续存在于应用阿片类药物的全过程，成为制约阿片类药物镇痛治疗的最大障碍，严重影响疾病的治疗及患者的生活质量。因此，OIC 已成为癌症治疗过程中亟需解决的重要问题，探究其有效防治手段是恶性肿瘤综合治疗中的一项重要任务。临床上迫切需要一种可长期使用、毒副作用少、疗效可靠的药物来填补这一领域的空白。

疏调通便方为云南省国医大师张震老基于“疏调气机”核心治法所创，从气论治便秘已取得良好的疗效;将疏调通便方用于治疗气滞腑实型阿片类便秘，以疏调气机为要，治疗阿片类药物所致的气滞腑实型便秘，疗效显著，并能显著改善其伴随的临床症状，提高患者生活质量。现报道如下。

1 资料与方法
