On the value of comfort nursing in transvaginal four-dimensional hysterosalpingo-contrast sonography

[Abstract] Objective To explore the value of comfort nursing in transvaginal four-dimensional hysterosalpingo-contrast sonography. Methods A total of 634 patients who underwent hysterosalpingography examination for infertility in the department of ultrasonic diagnosis of our hospital from January 2018 to December 2019 were selected.Out of the 634 patients，214 patients who received conventional nursing were set as the control group and 420 patients who received conventional nursing combined with comfort nursing were set as the observation group.The success rate，patient satisfaction and the incidence of adverse reactions （ARs） of hysterosalpingography examination in the two groups were compared and analyzed. Results The success rate of ultrasonography examination in the observation group was 98.33%，which was significantly higher than 93.46% in the control group，with statistically significant difference （P<0.05）. The patient satisfaction in the observation group was 99.05%，which was significantly higher than 95.33% in the control group，with statistically significant difference （P<0.05）. The incidence of serious ARs during and after operation in the observation group was 9.52%，which was lower than 18.22% in the control group，with statistically significant difference （P<0.05）. Conclusion Comfort nursing can improve the success rate of transvaginal hysterosalpingography and patient satisfaction，and reduce the incidence of ARs of operation.

隨着我国计划生育政策的开放，越来越多的育龄期妇女有了生育要求，而避孕措施的应用、计划生育手术及盆腔炎症均导致不孕症的增加。不孕症是生育期妇女常见的妇科疾病，其中因输卵管病变引发的不孕症逐年增加，占不孕症的25～30%[1]。因此是否能够准确评估子宫及输卵管功能成为治疗不孕症的关键。经阴道子宫输卵管四维超声造影 （Transvaginal four-dimensional hysterosalpingo-contrast sonography，TVS 4D-HyCoSy）是近年来新兴的评价输卵管通畅性的一项技术，因其能动态直观地显示子宫宫腔形态、输卵管走形及通畅性，并且具有安全、无放射性、价格低廉、可重复操作等优势，目前已广泛应用于临床并得到妇科医师的认可[2]。但是，该技术属于宫腔内操作，一些患者对该技术不了解，会产生焦虑、不安的情绪，因而影响超声造影检查的成功率及检查结果的准确性，我科室于2019年引入舒适护理技术，拟在生理、心理、社会及环境四个层面满足患者的需求，缩短患者不愉快的时间及程度，提高子宫输卵管超声造影检查的成功率、患者满意度，降低不良反应的发生率。本研究总结2018年1月至2019年12月期间我科室接受该超声造影检查的634例患者的检查配合情况及护理经验，现报道如下。

1 资料与方法

1.1 一般资料

选取2018年1月至2019年12月间因不孕于我院妇科就诊并在我科行经阴道子宫输卵管超声造影检查的患者634例。其中2018年1—12月间214例患者采取常规护理，作为对照组，患者年龄24～43岁，平均（28.56±4.65）岁，共419条输卵管，不孕时间0.5～9年，平均（4.56±3.42）年。……
