[摘要]目的：研究面針疗法联合调Q激光治疗对黄褐斑面积、皮肤生活质量及皮损严重程度的影响。方法：将2016年6月-2019年6月笔者医院收治的96例黄褐斑患者随机分为观察组（面针疗法+调Q激光治疗，n=48）和对照组（调Q激光治疗，n=48），比较两组治疗后黄褐斑面积、皮肤病生活质量及皮损严重程度。结果：治疗后，两组疗效比较无统计学差异（P>0.05）;治疗后，两组皮肤病生活质量指数量表（DLQI）得分均较其同组治疗前显著上升（P<0.05），黄褐斑生活质量量表（MELA-SQOL）得分均较其同组治疗前显著下降（P<0.05），且观察组治疗后DLQI得分显著高于对照组（P<0.05），MELA-SQOL得分显著低于对照组（P<0.05）;治疗后，两组皮损部位光密度及累计光密度均较其同组治疗前显著下降（P<0.05），且观察组治疗后皮损部位光密度及累计光密度均显著低于对照组（P<0.05）。两组不良反应发生率比较无显著性差异（P>0.05）。结论：面针疗法联合调Q激光可有效改善黄褐斑皮损，提高患者生活质量，且治疗安全性高，值得临床推广应用。

Effect of Facial Acupuncture Combined with Q-Switched Laser on Chloasma Lesions and Patients' Quality of Life

Abstract： Objective To investigate the effects of face acupuncture therapy combined with Q-switched laser irradiation on melasma area， quality of life and severity index. Methods 96 patients with melasma treated in our hospital from June 2016 to June 2019 were enrolled， and equally divided into two groups， each with 48 cases. Observation group was given face acupuncture therapy combined with Q-switched laser irradiation， while control group only was given Q-switched laser irradiation. Then the melasma area， quality of life and melasma severity indexes of two groups were compared. Results The clinical efficacy had no significant difference between groups （P>0.05）. The dermatology life quality index （DLQI） score was significantly increased in both groups after treatment （P<0.05）， and the score was higher in observation group than in control group （P<0.05）. After treatment， the MELA-subjective quality of life （MELA-SQOL） score was significantly decreased in both groups （P<0.05）， and observation group had lower MELA-SQOL score than that in control group （P<0.05），the average optical density and integral optical density of lesion were significantly decreased in both groups （P<0.05）， and the decrease was more obvious in observation group than in control group （P<0.05）. The adverse reaction rate had no difference between groups （P>0.05）. Conclusion  Face acupuncture therapy combined with Q-switched laser irradiation can effectively and safely alleviate melasma pigmentation level and improve quality of life for patients with melasma， which is worthy of clinical application.

黄褐斑为面部色素沉着性疾病，其病程长、斑块顽固，治疗难度较高，激光技术是目前治疗各种斑块的主要手段，并在临床中取得了较好疗效[2]。面针疗法能建立大量皮肤微血管通道，清除皮肤深层毒素[3]。近年来，笔者科室采用调Q激光联合面针疗法治疗黄褐斑患者，取得了满意疗效，现将结果报道如下。

1  资料和方法

1.1 一般资料：采用随机数字表法将笔者医院2016年6月-2019年6月收治的96例黄褐斑患者分为观察组（面针疗法+调Q激光治疗，n=48）和对照组（调Q激光治疗，n=48）。观察组：年龄33～48岁，平均年龄（40.16±7.16）岁，病程0.6～12年，平均病程（5.03±1.12）年，皮损类型：面上部型16例，面下部型13例，蝶形型19例;对照组：年龄32～51岁，平均年龄（41.03±7.16）岁，病程1～10年，平均病程（4.89±1.34）年，皮损类型：面上部型17例，面下部型14例，蝶形型17例，两组患者一般资料比较无显著性差异（P>0.05），具有可比性。
