摘要：目的探討热敏灸配合刺络拔罐治疗肩周炎的临床疗效。方法将60例肩周炎患者纳入研究，采用随机数字表法分成2组。对照组30例予以热敏灸，观察组30例在对照组的基础上予以刺络拔罐。比较2组患者的临床疗效，视觉模拟评分（VAS）与肩关节功能评分（JOA），肩关节活动度。结果观察组治疗总有效率为86.7%，高于对照组，差异有统计学意义（P<0.05）;治疗后2组患者VAS评分下降，JOA评分上升，观察组两项评分优于对照组，差异有统计学意义（P<0.05）;治疗后2组肩关节活动度均有增大，观察组内旋、外旋、外展、后伸活动度均大于对照组，差异有统计学意义（P<0.05）。结论热敏灸配合刺络拔罐治疗肩周炎的效果理想，可有效改善肩部疼痛，提高肩关节功能，值得临床推广。

Clinical Observation on the Treatment of Scapulohumeral Periarthritis Patientswith Heat

Sensitive Moxibustion Combined with Blooding-letting Puncture and Cupping

【Abstract】Objective： To study the clinical effect of heat-sensitive moxibustion combined with blooding-letting puncture and cupping in the treatment of scapulohumeral periarthritis patients. Methods： 60 patients were included in the study and divided into two groups by a random number table method， 30 cases per group. The control group was given heat-sensitive moxibustion， and the observation group was given blooding-letting puncture and cupping on the basis of the control group. The clinical effect， visual analog scale （VAS） and shoulder joint function score （JOA）， and range of motion of shoulder joints were compared between the two groups. Results： The total effective rate of treatment in the observation group was 86.7%， which was higher than that of the control group， and the difference was statistically significant （P<0.05）. After treatment， the VAS score of the two groups decreased and the JOA score increased. The two scores of the observation group were better than those of the control group， and the difference was statistically significant （P<0.05）. The range of motion of the shoulder joints in the two groups increased after treatment， and the range of motion of internal rotation， external rotation， abduction and extension in the observation group was greater than that of the control group， and the difference was statistically significant （P<0.05）. Conclusion： The combination of heat-sensitive moxibustion and blooding-letting puncture and cupping has an ideal effect on the treatment of scapulohumeral periarthritis patients， which effectively relieve shoulder pain and improve shoulder joint function， worthy of clinical application.

肩周炎是指由于外伤、受凉、年老体弱等原因，导致肩关节周围软组织发生退行性病变，肩关节发生慢性劳损的常见的炎症性疾病[1]。肩周炎患者以肩关节周围疼痛及功能活动受限为主要症状，目前尚无根治性疗法，而中医近年来在该领域的成果较为丰富。热敏灸可使处于敏化态的穴位呈现“小刺激大反应”，激发艾性感传，产生透热、扩热等现象，有利于调整机体功能，增强免疫能力。刺络拔罐是将刺络放血疗法与拔罐法有机结合起来，具有双重治疗作用，起到活血散瘀止痛，行气祛邪通络的目的[2]。单一治疗的效果终究是有限的、不确切的，因此本研究应用热敏灸配合刺络拔罐治疗肩周炎，以探讨其联合疗效。

1资料与方法

1.1一般资料将2018年1月—2020年1月于本院就诊的60例肩周炎患者纳入研究，用随机数字表法分成2组，各30例，并通过本院医学伦理委员会的审批。对照组男16例，女14例;年龄40～65岁，平均（52.3±3.6）岁;病程1个月～5 a，平均（2.2±0.7）a。……
