【摘 要】 目的：觀察 “阴中隐阳”法针刺风池穴为主治疗后循环缺血性眩晕的临床效果。方法：选取100例眩晕患者随机分“阴中隐阳”针刺法+长春西汀组（观察组）、常规针刺法+长春西汀组（对照组），每组各50人，观察比较治疗前后中医症候疗效、治疗前后眩晕症状好转程度（DARS和DHI）及经颅TCD检测椎-基底动脉血流动力学改变。结果：两组症候评分、两组DARS及DHI量表评分较治疗前均明显下降，且观察组下降明显（P<0.05）。两组椎-基底动脉最大血流速度较治疗前明显增加，且观察组增加明显（P<0.05）。观察组总有效率为90.00%，对照组为75.61%，观察组明显优于对照组（P<0.05）。结论：采用“阴中隐阳”法针刺双侧风池穴联合长春西汀治疗后循环缺血性眩晕的效果明显，值得临床推广应用。

Clinical Observation on the Treatment of 40 Cases of Posterior Circulation Ischemic Vertigo with Western

Medicine Combined with “Yang Hidden in Yin”Manipulation Acupuncture at Fengchi（G20） point

Abstract：Objective Observe the effect of “Yang Hidden in Yin” Acupuncture technique on bilateral Fengchi acupoint in the treatment of posterior circulation ischemic vertigo.Methods 100 patients with vertigo were randomly divided into two groups：“Yang Hidden in Yin” Acupuncture+Vinpocetine（Observation Group），ordinary acupuncture +Vinpocetin（Reference group），50 per group.The efficacy of TCM symptoms before and after treatment was observed and compared， the improvement of vertigo symptoms in each group before and after treatment （DARS and DHI vertigo rating scale） and the changes of vertebral - basilar blood flow dynamics were detected by transcranial TCD.Results 1.The symptom scores of the two groups and the scores of DARS and DHI in the two groups were significantly lower than before treatment， and the observation group was significantly lower（P<0.05）.2. The maximum blood flow velocity of vertebrobasilar artery in the two groups was significantly increased compared with that before treatment， and the observation group was significantly increased（P<0.05）.3. The total effective rate of the observation group was 90.00%， the control group was 75.61%， the observation group was significantly better than the control group （P<0.05）.Conclusion The effect of “Yin zhong yin yang” Acupuncture technique on bilateral Fengchi acupoint in the treatment of posterior circulation ischemic vertigo is better than that of Ordinary acupuncture manipulation and vinpocetine alone.

后循环缺血，一般指的是椎动脉、大脑后动脉和基底动脉构成的椎基底动脉系统的缺血，多由动脉粥样硬化、管腔狭窄或闭塞、动脉栓塞等因素造成;主要出现眩晕，头面部及肢体麻木感，乏力，恶心呕吐，视力改变，站立不稳或摔倒的临床症状[1]。因后循环缺血其病情反复，治疗效果不佳而一直被医药学界关注。目前临床给予溶栓、抗凝、改善脑部供血等西医常规治疗，但是治疗方案缺乏个体针对性，而且效果往往不尽如人意，加之其药物本身的副作用，限制了部分患者的使用;手术和血管内治疗虽有肯定的临床疗效，但是目前还有待研究，且手术风险大、创伤大、费用高昂、并发症较多，目前难以在基层普及[2]。……
