[摘要]随着自体脂肪移植技术的逐渐成熟，该技术被广泛应用于整形外科的各个领域。乳腺癌切除术后，自体脂肪移植用于即刻乳房重建也得到了初步应用，取得了良好的美学效果，且并发症较低，但安全性仍然是阻碍该技术用于即刻乳房重建的重要因素。目前，并没有明确的证据证明脂肪移植会增加肿瘤的复发率，本文就近年来自体脂肪移植在乳腺癌术后即刻乳房重建中的研究进展进行综述。

Abstract： With the gradual maturity of autologous fat transplantation technology， this technology has been widely applied in various fields of plastic surgery. Autologous fat transplantation for immediate breast reconstruction after breast cancer resection has also been preliminarily applied， with good aesthetic effects and low complications. However， safety is still an important factor that impedes the use of this technique for immediate breast reconstruction. Currently， there is no clear evidence that fat transplantation can increase the recurrence rate of tumors. This article reviews recent advances in research on immediate breast reconstruction after breast cancer with body fat transplantation.

近年来，随着三级预防的深入开展及诊断技术的不断提高，全球女性乳腺癌发病率及早期乳腺癌的查出率持续上升，严重威胁着女性的身心健康。同时，生物-心理-社会医学模式概念的转变，使乳腺癌术后乳房重建的安全性及美学效果受到更多医生和女性患者的关注。自体脂肪移植是一种新兴的、很有前景的治疗技术，无论是单纯脂肪移植还是联合背阔肌皮瓣转移技术，其在延迟乳房重建或即刻乳房重建中都获得了良好的美学效果，且并发症较低[1]。

1  自体脂肪移植技术

自体脂肪移植技术主要包括脂肪获取、脂肪移植准备及脂肪注射三个过程。

1.1 脂肪获取：手持注射器吸脂术、抽脂术和超声辅助抽脂术是目前常用的获取脂肪的方法，有研究表明[2-4]，手持注射器吸脂术对脂肪组织的创伤较小，脂肪细胞成活数目最多，生存能力更强;当脂肪细胞置于一种高压环境时，脂肪细胞活性下降，分裂能力降低[4]，而手持注射器吸脂术抽吸压力小，脂肪细胞易存活。……
