[摘要] 目的 探讨分析糖尿病下肢动脉硬化闭塞症患者接受CT诊断的临床效果，对CT应用价值进行分析，给临床诊断提供依据。 方法 2014年4月—2015年11月该院对糖尿病下肢动脉硬化闭塞症患者68例开展了研究分析，为这些患者提供了数字减影血管造影和双螺旋CT诊断，对两种检查方式的临床效果进行了比较研究。 结果 糖尿病下肢动脉硬化闭塞症患者接受CT诊断的敏感度是85.45%，特异度是61.54%，准确性是80.88%，误诊率是38.46%，漏诊率是14.55%，阳性预测值是90.38%，阴性预测值是50.00%。全部患者均能够顺利完成双螺旋CT检查，患者的血管显影清晰，没有伪影存在，全部共研究分析了1621段血管，有轻度狭窄血管458段，中度狭窄血管264段，重度狭窄血管125段，血管壁钙化和斑块化血管共有1 412段。 结论 糖尿病下肢动脉硬化闭塞患者选择双螺旋CT检查能够获得较好的效果，检出率高，误诊率低，临床中应该进行推广使用。

Analysis of Application of Double Spiral CT in Diagnosis of Diabetic Lower Extremity Arteriosclerosis Obliterans

[Abstract] Objective To study and analyze the clinical effect of double spiral CT in diagnosis of Diabetic lower extremity arteriosclerosis obliterans， analyze the application value of CT thus providing basis for the clinical diagnosis. Methods 68 cases of patients with diabetic lower extremity arteriosclerosis obliterans in our hospital from April 2014 to November 2015 were selected for research and analysis， and the clinical effect of the two examination ways was compared and researched. Results The sensitivity， specificity， accurate rate， misdiagnosis rate and missed diagnosis rate， positive predictive value and negative predicative value of the patients after the CT diagnosis were respectively 85.45%， 61.54%， 80.88%， 38.46%， 14.55%， 90.38%， 50.00%， and all patients could successfully finish the double spiral CT study， and the blood vessel of patients was clear， without constructed defect， and 1 621 blood vessels were analyzed， including 458 mild stenosis blood vessels， 264 moderate stenosis blood vessels， and 125 severe stenosis blood vessels， and there were 1 412 angiolithic degeneration and atherosclerotic vessels. Conclusion The effect of double spiral CT in diagnosis of diabetic lower extremity arteriosclerosis obliterans is better with high test rate and low misdiagnosis rate， which can be promoted and applied in clinic.
