[摘要] 目的 探討圆锥角膜应用角膜胶原交联治疗的临床效果。方法 方便选择该院2015年5月—2016年5月接收的圆锥角膜患者41例，均给予角膜胶原交联治疗，观察治疗效果。结果 ①不适症状：术中（2.93±0.58）分，术后1 d （2.71±0.62）分，术后3 d （1.53±0.67）分，术后1周（0.38±0.47）分，术后1个月（0.30±0.31）分；术后3 d、术后1周、术后1个月低于术中及术后1 d，且术后1周、术后1个月低于术后1 d（t=6.147、6.338、6.534、7.264、7.301，P<0.05）。②末次随访时，19.5%裸眼视力、24.4%最佳矫正视力提高2行以上。③术前，SmiK（6.08±3.61）、Kmin（47.08±2.34）、中央角膜厚度（468.23±28.91）μm、最薄处角膜厚度（447.82±31.82）μm、角膜内皮细胞（2 798.17±272.07）个/mm2，术后1年SimK（5.51±3.07）、Kmin（46.87±2.49）、中央角膜厚度（467.15±24.65）μm、最薄处角膜厚度（439.05±35.78）μm、角膜内皮细胞（2 816.63±279.76）个/mm2，（t=0.528、0.824、0.647、0.627、0.539，P>0.05）；术前Kmax（54.28±4.22）、术后1年Kmax（52.67±4.62），（t=6.301，P=0.015）。结论 圆锥角膜应用角膜胶原交联治疗时，具有良好的治疗效果，可有效改善患者角膜及视力情况。

The Effect of Corneal Collagen Cross-linking on Keratoconus

[Abstract] Objective This paper tries to explore the therapeutic effect of clinical application of keratoconus corneal collagen crosslinking. Methods 41 patients with keratoconus were convenient selected in this hospital from May 2015 to May 2016， were given cross-linking of corneal collagen treatment， the treatment effect was observed. Results ①The symptoms of discomfort were： intraoperative （2.93±0.58）points， postoperative 1 d （2.71±0.62）points， postoperative 3 d （1.53±0.67）points， postoperative 1 week （0.38±0.47）points， postoperative 1 month（0.30±0.31）points； 3 d after surgery， 1 week after surgery， 1 month after surgery was lower than intraoperatively and 1 day after operation， and 1 week after operation and 1 month after operation were lower than 1 day after operation（t=6.147， 6.338， 6.534， 7.264， 7.301， P<0.05）. ②the last follow-up，19.5% uncorrected visual acuity， 24.4% best corrected visual acuity increased more than 2 lines. ③The preoperative values of SmiK （6.08±3.61）， Kmin （47.08±2.34）， central corneal thickness （468.23±28.91） μm， thinnest corneal thickness（447.82±31.82） μm and corneal endothelial cells（2 798.17±272.07）mm2， ofter 1year Simk（5.51±3.07），Kmin （46.87±2.49）， the central corneal thickness（467.15±24.65）μm， the thinest corneal was （439.05±35.78）μm， the corneal endothelial cells（2816.63±279.76）mm2（t=0.528，0.824，0.647，0.627，0.539， P>0.05）；preoperative Kmax （54.28±4.22）， 1 year after surgery Kmax （52.67±4.62）（t=6.301，P=0.015）. Conclusion The application of keratoconus corneal collagen crosslinking treatment has good treatment effect， which can effectively improve the cornea and visual acuity in patients.

青少年为圆锥角膜高发人群，双眼通常会先后进行性发病，疾病进展具有渐进性，可在任何年齡终止，形成瘢痕，显著减退视力[1]。目前，临床上存在较多的治疗圆锥角膜的方法，但多数方法仅能将屈光不正纠正，无法对病情进展做出有效的阻止。近几年研究指出，角膜胶原交联法，即紫外线A照射与核黄素联合治疗法在圆锥角膜治疗中能够获得较好的治疗效果。因此，该院在2015年5月—2016年5治疗41例圆锥角膜患者时，即采用角膜胶原交联法，疗效较理想，现报道如下。

1 资料与方法

1.1 一般资料

方便选择该院接收的圆锥角膜患者41例，男26例，女15例；年龄18～31岁，平均（23.7±4.3）岁；病程2个月～4年，平均（1.9±1.2）年；裸眼视力0.09～0.28，平均（0.15±0.08）；散光-5.00～-9.00D，平均（-6.34±-0.14）D。纳入标准：①与圆锥角膜诊断标准相符合，且处于进展期；②角膜上皮刮除后，角膜厚度400 μm以上；③瘢痕未明显存在于角膜基质层；④经伦理委员会批准，且知情同意；⑤排除伴有全身风湿免疫类疾病患者。……
