[摘要]目的 比较经子宫内膜息肉电切术（TCRP）后应用左炔诺孕酮宫内节育系统（商品名：曼月乐环）和去氧孕烯炔雌醇片（商品名：妈富隆）在预防子宫内膜息肉复发中的效果。方法 选取2014年1月～2016年1月我院收治的180例子宫内膜息肉TCRP术后患者，采用随机数字表法将其分为观察组（放置曼月乐，n=90）和对照组（口服妈富隆3～6个月，n=90）。观察比较两组患者术后月经量、子宫内膜厚度变化以及子宫内膜息肉复发情况。结果 治疗后6、12个月，观察组月经量[（46.07±15.79）、（26.97±10.35）ml]和复发率（1.11%）均显著低于对照组[（71.12±19.27）ml、（85.72±20.28）ml、14.44%]，差异有统计学意义（P<0.05）。治疗后6个月，两组患者子宫内膜厚度变化差异无统计学意义（P>0.05），但治疗后12个月，观察组子宫内膜厚度显著低于对照组，差异有统计学意义（P<0.05）。结论 相较于口服妈富隆，TCRP术后应用曼月乐能够更有效地预防子宫内膜息肉复发。

Effect comparison of Mirena and Marvelon in the prevention of endometrial polyp recurrence after TCRP

[Abstract]Objective To compare the effect of levonorgestrel-releasing intrauterine system （trade name：Mirena） and Desogestrel and Ethinylestradiol Tablets （trade name：Marvelon） on the prevention of the recurrence of endometrial polyps after TCRP.Methods From January 2014 to January 2016，180 patients with endometrial polyps treated by TCRP in our hospital were selected and divided into observation group （Mirena placement，n=90） and control group （oral administration of Marvelon for 3 to 6 months，n=90） in random number table.The changes of menstrual blood volume after surgery，endometrial thickness，and endometrial polyp recurrence were observed and compared in the two groups.Results 6- and 12-month after treatment，the menstrual blood volume and recurrence rate in the observation group were （46.07±15.79） ml，（26.97±10.35） ml，and 1.11% respectively，was significantly lower than that in the control group [（71.12±19.27） ml，（85.72±20.28） ml，14.44%]，the difference was statistically significant （P<0.05）.After 6-month therapy，there was no significant difference in the thickness of endometrium between the two groups （P>0.05）.However，12-month after treatment，the endometrium of the observation group was greatly thinner than that in the control group，the difference was statistically significant （P<0.05）.Conclusion Compared with oral administration of Marvelon，application of Mirena can more effectively prevent the recurrence of endometrial polyps after TCRP.

子宮内膜息肉是一种复发率高，广泛见于青春期后各年龄段妇女的常见病变，这其中又以生育期妇女最为多见[2]。子宫内膜息肉多为子宫内膜局部的结缔组织和血管增生形成息肉样赘生物，临床上多表现为经量增多、经期延长、不规则阴道出血以及不孕等。近年来，随着宫腔镜技术的推广和应用，宫腔镜下子宫内膜息肉电切术（TCRP）成为了治疗子宫内膜息肉的首选方案，但其复发率仍旧较高，为了探究解决这一难题的方法，我院于2014年1月～2016年1月对180例TCRP术后患者应用左炔诺孕酮宫内节育系统和去氧孕烯炔雌醇片两种方式，全方面探究两者在预防子宫内膜息肉复发方面的疗效，现报道如下。……
