[摘要]目的 探讨“改良床上洗头法”在肝癌TACE治疗卧床患者中的应用效果。方法 选择2016年1～12月在本院进行TACE治疗的卧床肝癌患者80例，随机分为观察组与对照组，各40例。对照组患者采用传统“简易床上洗头盆洗头法”洗头，观察组采用“改良床上洗头盆洗头法”洗头，比较两组洗头效果及患者满意度。结果 观察组洗头时间为（15.12±2.41）min，床单浸湿范围为（6.51±1.23）cm×cm，明显优于对照组的（20.43±1.32）min和（30.15±3.56）cm×cm；观察组患者护理总满意率为95.0%，明显高于对照组的82.5%，差异有统计学意义（P<0.05）。结论 “改良床上洗头盆洗头法”不仅节约了护理人員的时间，更重要的是提高了患者的满意度，值得临床应用推广。

Application and observation of modified washing head in bed in liver cancer patients lying in bed treated with TACE

[Abstract]Objective To explore the application effect of modified washing head in bed in liver cancer patients lying in bed treated with transhepatic arterial chemotherapy and embolization （TACE）.Methods A total of 80 liver cancer patients lying in bed treated with TACE from January to December 2016 in our hospital were selected and evenly divided into observation group and control group in random，40 cases in each group.In the control group，traditional simplified washing head in bed was used，while in the observation group，the modified one was applied.The effect of washing head and patient′s satisfaction were compared.Results In the observation group，washing time was （15.12±2.41） min and wetting area of bed sheet was （6.51±1.23）cm×cm，greatly better than those in the control group of （20.43±1.32） min and （30.15±3.56）cm×cm respectively.The total nursing satisfaction by patients in the observation group was 95.0%，much higher than that in the control group accounting for 82.5% with a statistical difference （P<0.05）.Conclusion Modified washing head in bed not only saves nursing staff′s time，also improves patient′s satisfaction，which is worthy of application and promotion in clinic.

如今，随着优质护理服务在临床上的开展，护理人员需要为生活不能自理的卧床患者提供如洗头、擦身等基础护理服务，加上患者对自身舒适度和生活质量的追求，头部的清洁护理已成为护理人员的常规工作[1]，而床上洗头则是头部护理的重要项目之一。传统床上洗头法用量杯手动淋水，需要护理人员反复弯腰取水，增加护理人员肩、腰痛的机会，水量大小不容易控制而使水容易进入患者双耳及眼，且容易浸湿床单及患者衣领，造成患者不适，增加用水量[2]，因此，我科护理人员从患者的角度考虑，从实际情况出发，结合临床经验将“简易床上洗头盆洗头法”进行改良，用一次性灌肠袋+橡胶管供水取代用量杯手动淋水，该洗头法应用于临床后，取得较好的效果，现报道如下。

1 资料与方法
