试管凝集试验与虎红平板凝集试验在布鲁氏杆菌病诊断中应用价值比较

[摘要] 目的 比较试管凝集试验与虎红平板凝集试验在布鲁氏杆菌病诊断中应用价值。方法 方便选取济南市传染病医院及解放军第404医院普外科2014年8月—2016年8月收治的128例布鲁士氏杆菌病患者为研究对象,均取静脉血,再行试管凝集、虎红平板凝集试验,比较两种检测方法结果。 结果 该组128例患者血清中,结果显示:RBPT呈阳性24例(18.75%);SAT呈阳性20例(15.63%),其中1∶100有10例,1∶100以上有10例,可疑6例,两种检测方法的阳性符合率为93.81%,试管凝集试验与虎红平板凝集试验的阳性检出率比较差异无统计学意义(P>0.05),试管凝集试验特异性高,虎红平板凝集试验敏感性高。结论 试管凝集试验可用于确诊初筛阳性及诊治临床患者,虎红平板凝集试验可用于初筛、调查流行病;若结合两种方法用于诊断布鲁氏杆菌病,可提高阳性检出率,避免假阳性。

Comparison of Application Value of Tube Agglutination Test and Rpst in Diagnosis of Brucellosis

[Abstract] Objective To compare the application value of tube agglutination test and rpst in diagnosis of brucellosis. Methods 128 cases of patients with brucellosis admitted and treated in our hospital from August 2014 to August 2016 were convenient selected as the research objects and the vein blood was extracted for tube agglutination test and rpst, and the test results of the two methods were compared. Results In the serum of 128 cases of patients, the results showed that the RBPT of 24 cases was positive, accounting for 18.75%, the SAT of 20 cases were positive, accounting for 15.63%, 10 cases were 1∶100 and 10 cases were above 1∶100 and 6 cases were suspected, the positive coincidence rate of the two methods was 93.81%, and the difference in the positive test rate between the tube agglutination test and rpst had no statistical significance(P>0.05), the specificity of tube agglutination test was high and the sensitivity of rpst was high. Conclusion The tube agglutination test can be used to diagnose the clinical positive patients, and the rpst can be used for preliminary screening and survey of epidemic diseases, and the application of the combination of the two methods in diagnosing the brucellosis can improve the positive test rate and avoid the false positive results.

布魯氏杆菌病是一种传染-变态反应性传染病,由布鲁氏菌属细菌入侵人体所引起,属人畜共患疾病[1]。人体内的布鲁氏菌病来源于动物传播,若人类患有该病,可造成公共卫生问题。布鲁氏菌病的传播根除关键在于扑杀患病动物,切断传播源及灭杀传染源。因此,寻求布鲁氏杆菌病的准确、快速的检测方法,有助于疾病的防治,控制疾病发生与流行。该研究旨在分析试管凝集试验与虎红平板凝集试验用于诊断布鲁氏杆菌病的效果,现选取2014年8月—2016年8月于济南市传染病医院、解放军第404医院收治的128例布鲁士氏杆菌病患者临床资料为研究对象,现报道如下。 ……
