[摘要] 目的 通过开展处方点评，提高处方质量，促进合理用药。方法 分析北京市大兴区妇幼保健院2015年1～12月全部点评处方180 993张，经电子处方点评系统并辅以人工点评，对处方的规范性、用药不适宜性及超常处方进行归纳分析。结果 处方不合格率为1.37%，不合格处方中，不规范处方占53.08%，用药不适宜处方占46.92%，无超常处方。结论 完善处方点评的信息化系统，加强点评人员的药学和临床专业学习，提高不合理处方的发现率，以便在临床工作中进一步改进，提高合理用药水平。

The role of prescription comment on the whole samples in rational drug use

[Abstract] Objective To improve quality of prescriptions and rational use of medicines through prescriptions comments. Methods All 180 993 prescriptions from January to December 2015 were commented for an evaluation in respect of prescription standardization， inappropriate use of drugs and the unusual prescription by the combination of electronic prescription review system and manual reviews. Results Among all the prescriptions， the rates of unqualified prescriptions were 1.37%. Among them， irregular prescriptions accounted for 53.08%， 46.92% of the prescription was not appropriate， there was no unusual prescription. Conclusion Improving the information system of prescription review， strengthening the pharmacy and clinical evaluation of personnel， and improving the detection rate of unreasonable prescriptions， will help to improve the level of rational drug use.

醫疗机构应建立处方点评制度，对处方实施动态监测和超常预警，是《处方管理办法》规定的，其目的是促进药物的合理使用和保障患者的用药安全。处方是医师实施药物治疗的重要文书，对处方书写的规范性及临床应用进行评价，发现存在或潜在的问题，制定并实施相应的干预和改进措施，促进临床药物合理应用的过程[1]，通过全样本的处方点评，有利于发现微小问题，以提高药物的有效性、安全性、经济性，促进药物的合理使用。

1 资料与方法

1.1 处方来源

将北京市大兴区妇幼保健院2015年1～12月全部处方180 993张，进行处方点评，将不合理处方通过处方点评信息系统导出，生成Execl表格，统计出处方医师、科室、具体不合格项目。全体药师对纸质处方再一次点评，通过Execl表格记录科室、不合格处方内容，将电子处方点评与纸质处方点评两者结合，确保全样本处方点评。……
