双源CT大螺距Flash扫描模式在胸腹主动脉CTA的应用

目的：利用双源CT大螺距Flash扫描模式对胸腹主动脉进行扫描成像，对照评价其图像质量和辐射剂量，探讨其应用价值。方法：将我院50例行胸腹主动脉CTA检查的患者随机分为A、B 2组，A组使用Flash大螺距扫描模式，B组使用常规扫描模式。对2种扫描模式下的主动脉图像质量和辐射剂量进行对比评价，客观评价包括图像噪声和信噪比。主观评价由2名放射科医生独立完成，辐射剂量统计使用设备自带的数据。结果：A组图像质量评分（2.21±0.58）低于B组（2.66±0.21），差异有统计学意义（P＜0.05）。客观图像质量对比无明显差异。扫描时间A组（2.3±0.10）s，比B组（7.8±0.90）s缩短，差异有统计学意义（P＜0.05）；A组的辐射剂量（2.9±0.6）mSv，对比B组（3.8±0.7）mSv，差异有统计学意义（P＜0.05）。结论：双源CT的Flash大螺距模式扫描胸腹主动脉时，可以获得清晰的主动脉图像，能满足胸腹主动脉病变的诊断；扫描时间短，辐射剂量少，在急诊主动脉大血管扫描检查有较高临床应用价值。

Abstract：［Objective］The use of high pitch dual source CT scan mode for Flash scan imaging of the thoracic and abdominal aorta，evaluating the image quality and radiation dose，to explore its application value.［Method］50 patientswith CTA in our hospitalwere random ly divided into group A and group B.Group A was used in Flash group，and B group was used in routine scan mode.Evaluation of the quality and radiation dose of aortic imageswas carrying out in two differentscanningmodes.Objective evaluation included image noise and signal to noise ratio.The subjective evaluation was done by 2 radiologists independently.Radiation dose statistics using the device comeswith the data.［Result］The image quality score（2.21±0.58）in group A was lower than that in group B（2.66±0.21），and the differencewas statistically significant（P＜0.05）.Therewas no obvious difference in objective image quality.The scanning time of group A（2.3±0.10）s，wasmore shortened than in the group B（7.8±0.90）s，the differencewas statistically significant（P＜0.01）；radiation dose in group A（2.9±0.6）mSv，compared with group B（3.8±0.7）mSv，was statistically significantdifferent（P＜0.01）.［Conclusion］Dual source CT Flash pitch scanningmode of thoracic and abdominal aorta，aortic could get clear image，could meet the diagnosis of abdominal aortic disease；shorten scan time，lower radiation dose，so ithas high clinicalapplication value in the emergency aorta scan.

胸腹主动脉疾病严重影响人类健康，主要包括 主动脉夹层、主动脉瘤、主动脉壁间血肿、动脉粥样硬化性溃疡和大动脉炎等。其中主动脉夹层发病急，进展快，具有较高的死亡率，其临床表现复杂，误诊率较高，因此提高主动脉夹层早期诊断的准确率尤为重要。随着螺旋CT技术的快速发展和扫描速度的提高，多层螺旋CT血管成像（CTA）在主动脉急症中的应用越来越广泛，已成为主动脉疾病的首选检查方法［1］。……
