二甲双胍治疗社区老年2型糖尿病患者的疗效及安全性

目的 探讨二甲双胍治疗社区老年2型糖尿病患者的临床效果和安全性。方法 选取社区老年糖尿病患者90例为研究对象，按随机数表法分为对照组和观察组，每组45例。对照组患者采用瑞格列奈治疗，观察组患者采用二甲双胍治疗，比较两组患者的血糖控制情况和不良反应发生情况。结果 观察组患者的治疗总有效率为95.6%，显著高于对照组的80.0%，差异有统计学意义（P＜0.05）；观察组患者的空腹血糖、餐后2 h血糖及糖化血红蛋白水平分别为（6.1± 1.0）mmol/L、（7.1±1.7）mmol/L、（9.4±2.0）%，均显著低于对照组的（8.0±1.6）mmol/L、（8.3±1.6）mmol/L、（10.7± 2.3），差异有统计学意义（P＜0.05）；观察组患者的不良反应发生率为6.7%，显著低于对照组的22.2%，差异有统计学意义（P＜0.05）。结论 二甲双胍治疗社区老年2型糖尿病有利于控制患者的血糖，且能够降低各类不良事件的发生风险，安全性高。

【Abstract】Objective To study the effect and safety of metformin in the treatment of elderly patients with type 2diabetes mellitus in community.Methods Select diabetic older 90 patients for the study，according to the random number table divided into control group and observation group，45 cases in each group. In the control group were treated with repaglinide treatment，the observation group were treated with metformin，glycemic control and adverse reactions between the two groups of patients were observed.Results Treatment of patients in the observation group total effective rate was 95.6%，significantly higher than 80.0%，the difference was statistically significant（P＜0.05）；fasting blood glucose，postprandial 2 h blood glucose and glycated hemoglobin levels were observed in patients（6.1± 1.0）mmol/L，（7.1±1.7）mmol/L，（9.4±2.0）%，significantly lower than the control group（8.0±1.6）mmol/L，（8.3±1.6）mmol/L，（10.7±2.3），the difference was statistically significant（P＜0.05）；adverse reactions observed in patients incidence of 6.7%，significantly lower than the 22.2% in the control group，the difference was statistically significant（P＜0.05）.Conclusion Metformin community elderly type 2 diabetes patients help control blood sugar，and can reduce the risk of various types of adverse events，safe.

随着社会经济水平的不断发展和人们生活水平的提升，社区糖尿病的发病率也呈逐年上升趋势［1-2］，加上老年患者各脏器存在不同程度的功能受损，很容易由于血糖代谢紊乱而导致2型糖尿病发生。患者一旦确诊为糖尿病，不仅会存在长期血糖上升症状，同时还会伴随不同程度的并发症，对患者的生命质量产生不良影响。临床治疗中对患者血糖进行有效控制的同时，也要注意对并发症给予针对性治疗［3］，为患者血糖控制和生命质量提升创造有利条件。本研究就二甲双胍治疗社区老年2型糖尿病患者的临床效果进行分析，并观察其治疗的安全性，现报道如下。……
