摘 要 目的：通过对全科医师技能培训的需求调查，筛选出中心全科医师最迫切希望提升的8项技能，经过培训与考核，让全科医生基本掌握8项技能。方法：对中心30名45周岁以下全科医师进行技能需求问卷调查，筛选出需求前8项的技能，以2个月1项技能的速度进行强化培训。结果：30名参加培训的全科医师培训前、后成绩差异有统计学意义（P<0.01），技能明显得到提升。结论：基于社区卫生服务中心实际需求的培训能大幅度提高全科医师的操作技能，满足社区卫生服务中心临床需求。

Evaluation of the effect of clinical skill intensive training for general practitioner based on demand

ABSTRACT Objective： Through the investigation of the demand of the skill training of general practitioners（GP）， the eight skills of general practitioners which were urgent to be enhanced were selected in the center. After training and assessment， it made GP master the eight skills. Methods： A questionnaire survey was conducted on the skill requirement in 30 general practitioners under 45 years of age， the top 8 skills required were screened out and 1 skill of them was intensively trained at the speed for 2 months. Results： There were statistically significant differences in the results before and after the training in the 30 participants （P<0.01）， and their skills were improved significantly. Conclusion： The training based on the actual demand of the community health service center can greatly improve the skills of GP， and satisfy the clinical needs of the community health service center.

全科医疗实践活动显示，一名合格的全科医生，除了应当具备坚实的医学基础知识外，还必须具备专业的临床技能[1]。上海市徐汇区曾经做过相关调查，社区全科医师认为自己需要补充的知识和技能中，被选中频率最高的3类知识和技能依次为：临床实践技能（69.97%），临床理论知识（42.32%）和社区工作经验（41.64%）[2]。2012年起，我中心全面推行家庭医生制服务及开设老年护理病房，家庭病床及护理病房病情复杂，需要鼻饲、导尿、静脉留置针的老年患者非常多，全科医师技能操作水平较低这一问题逐渐凸显，如何在短期内提升全科医师临床技能迫在眉睫。针对这一现状，2014—2015年我中心对30名45周岁以下全科医师进行了为其2年的技能强化训练，现对训练情况进行总结、评价，旨在为社区临床技能的提升积累经验。

1 对象与方法

1.1 培训对象

我中心参加家庭医生制服务及老年护理病房工作的全部全科医生，共30名，年龄均在45周岁以下。

1.2 培训内容

从临床基本操作技能中选取了20项社区应用较多的技能，对30名全科医生进行调查问卷，结合家庭医生制服务及老年护理病房的实际工作需求选出至少8项目前全科医生比较薄弱且急需掌握的技能（表1）。……
