摘 要 本文对上海市徐汇区在分级诊疗方面的做法进行介绍。徐汇区区域内有丰富的医疗卫生资源，通过完善优质的社区卫生服务体系，借助信息化手段，推进慢性病长处方、医联体等试点，积极探索区域内分级诊疗可行的政策支持和技术路线。

Practice and exploration of the hierarchical medical system in Xuhui District

ABSTRACT This article introduces how Xuhui District probes the hierarchical medical system. There is a wealth of medical and health care resources in the Xuhui District Region. Through the improvement of the excellent community health service system， with the help of information technology， and pilot projects of promotion of the long-term prescription for chronic diseases and medical conjoined hospitals， Xuhui District actively explores the feasible policy support and technical route for the hierarchical medical system in the region.

国务院办公厅在《关于推进分级诊疗制度建设的指导意见》（国办发〔2015〕70号）中明确提出了建立分级诊疗制度的原则[1]：基层首诊、双向转诊、急慢分治、上下联动。上海市徐汇区不断深化社区卫生服务综合改革，注重资源整合，注重制度创新，重视实际应用，借助信息化手段，进一步做实家庭医生签约服务，夯实社区基础，在推进区域分级诊疗制度建立方面进行了有益的探索和实践。

1 丰富的区域医疗卫生资源是推进分级诊疗的基础

徐汇区具有完善的社区卫生服务体系[2]，13家社区卫生服务中心、73个社区卫生服务站、7家上海市示范社区卫生服务中心、3家全国示范社区卫生服务中心共同形成了“步行15 min就医圈”。

1.1 优质从业人员

徐汇区2014年全科临床医生439名（高级职称16%，中级职称73%），每万人全科医生超过4名。社区卫生服务中心门诊量达到536万人次，开放床位1 154张，出院4 690人次，床位使用率100.7%；家庭病床4 487张，上门服务人次7.6万人次。

1.2 优质医疗卫生资源

徐汇区拥有三级甲等医院8家，二级甲等综合性医院2家，二级乙等综合性医院1家，民办医院12家。2014年区域内各医疗机构门、急诊总量2 494万人次，住院手术29.9万人次，出院48.6万人次。……
