甘草人参止血复方治疗老年非静脉曲张性上消化道出血的临床疗效观察

目的观察甘草人参止血复方治疗老年非静脉曲张性上消化道出血的临床疗效，并比较不同剂型的疗效差异。方法选择2013年1月～2015年12月收治的101例老年非静脉曲张性上消化道出血患者，将其随机分为三组：中药饮片组（A组，n=33）、中药免煎颗粒剂组（B组，n=34）、云南白药组（C组，对照组，n=34），三组患者均给予基础治疗，A组、B组均服用甘草人参止血复方治疗，C组予云南白药治疗，观察疗程7 d。结果A、B、C组患者的愈显率分别为45.46%、44.12%、47.06%，总有效率分别为96.97%、97.06%、97.06%，愈显率和总有效率相当；A组患者的中医证候疗效为54.36%，优于B组和C组，而B、C组患者的出血停止时间、平均住院天数、平均住院费用均优于A组。结论甘草人参止血复方是治疗老年非静脉曲张性上消化道出血患者的有效方药，中药饮片普通煎剂在中医症候改善方面有一定优势，而颗粒剂便于急诊患者的临床应用。

［Abstract］Objective To observe the clinical effects of Liquorice-Ginseng Haemostatic Compound in treatment of nonvariceal gastrointestinal bleeding in elderly patients，and to compare the effects of different formulations.M ethods 101 elderly patientswith non-variceal gastrointestinal bleeding treated in our hospital from January 2013 to December 2015 were selected and randomly divided into three groups，namely the TCM decoction pieces group（n=33，group A），the TCM herbal concentrate-granules group（n=34，group B），and the Yunnan Baiyao group（n=34，group C，control group）. Patients in three groupswere all given fundamental treatment，and those in group A and group B were given Liquorice-Ginseng Haemostatic Compound，while those in group C were given Yunnan Baiyao，for a treatment course of 7 d.Results The complete response rateswere 45.46%，44.12%，and 47.06%in group A，B，and C，respectively，and the overall response rateswere 96.97%，97.06%，and 97.06%，which were comparable across the three groups.The TCM syndrome effects in group A was 54.36%，significantly higher than those in group B and group C.However，the bleeding duration，average hospital stay，and average hospitalization costs in group B and group C were superior to those in group A.No obvious adverse reaction was observed in three groups.Conclusion Liquorice-Ginseng Haemostatic Compound has favorable effects in treatment of non-variceal gastrointestinal bleeding in elderly patients.TCM decoction pieces have an advantage in improvement of syndromes of Chinesemedicine，while the TCM herbal concentrate-granules are convenient for emergency treatment.
