补肾泄毒法治疗尿毒症并发急性左心衰临床观察*

目的 观察补肾泄毒法治疗对尿毒症并发急性左心衰患者肾功能和血液流变学和功能指标的影响。方法 130例患者按照随机数字表法分为对照组和治疗组各65例。对照组给予西医常规治疗，治疗组在对照组基础上给予自拟补肾泄毒方治疗，比较两组临床疗效、肾功能指标、血液流变学指标、心功能指标和不良反应发生情况。结果 治疗组总有效率89.23%高于对照组的72.31%（P＜0.05）。治疗后两组血尿素氮（BUN）、血肌酐（Cr）及内生肌酐清除率（Ccr）与治疗前比较均改善（均P＜0.05），且治疗组BUN、Cr下降，Ccr升高均优于对照组（均P＜0.05）。治疗后两组全血黏度、血浆黏度、红细胞压积及红细胞聚集指数均明显低于治疗前，且治疗组优于对照组（均P＜0.05）。治疗后两组左室舒张末径（LVDEd）、左室收缩末径（LVSEd）、左房内径（LAd）与治疗前比较均降低，且治疗组优于对照组（均P＜0.05）。治疗组的不良反应发生率为10.77%，明显低于对照组的26.15%（P＜0.05）。结论 补肾泄毒法治疗尿毒症并发急性左心衰患者，可提高总有效率，改善患者的肾功能、血液流变学指标和心功能指标，不良反应轻。

【Abstract】Objective：To discuss the effects of Bushen Xiedu Decoction on the indexes of renal function，blood rheology and heart function in patients with uremia complicated by acute left heart failure.Methods：130 cases were randomly divided into the control group and the treatment group，65 cases in each.The control group were given conventional western medicine treatment，while the treatment group took extra Bushen Xiedu Decoction.The indices of renal function，blood rheology and heart function and the occurrence of adverse reactions were compared.Results：The total effective rate of the treatment group was 89.23%，higher than that of the control group 72.31%（P＜0.05）.compared with before treatment，BUN，Cr and Ccr improved in both group after treatment（P＜0.05），and the drop of BUN and Cr and the rise of Ccr in the treatment group were superior to those in the conrol group（P＜0.05）.After treatment，whole blood viscosity，plasma viscosity，hematocrit and red cell assembling index were obviously lower than those before treatment，and the treatment group were superior to the conrol group（P＜0.05）.Compared with before treatment，LVDEd，LVSEd，LAd dropped in both groups after treatment，and the treatment group were superior to the conrol group（P＜0.05）.The incidence of adverse reaction in the treatment group was 10.77%，obviously lower than that of the control group 26.15%（P＜0.05）.Conclusion：Bushen Xiedu method on uremia complicated by acute left heart failure can raise the total effective rate，improve the indices of renal function，blood rheology and heart function，with slight adverse reaction.
