综合医院心理科两年会诊转入患者的特征回顾性分析

目的研究综合医院临床心理科会诊联络转入患者的特征。方法回顾性分析我院心理科近两年通过联络会诊转入患者的数据，共收集了68例患者，分析其来源科室分布、转入后的诊断种类、转归情况。结果转入前三位的科室为神经内科、消化科、心内科，分别为22.1%、19.1%、16.2%；最后诊断为前三位的疾病为：焦虑症、抑郁症、躯体疾病所致的精神障碍，分别占20.6%、16.2%、14.7%。治愈48.5%，好转48.5%，最后的总有效率为97.0%。结论转入患者反映了疾病谱的改变和精神卫生事业的发展，说明心理科与其他综合科在转诊患者的合作密切程度，进一步说明了在综合医院设立临床心理科的重要性。

［Abstract］Objective To study the features of patients referred to psychological department of a general hospital by consultation and contact.Methods Materials of patients referred to psychological department of a general hospital by consultation in recent two years were reviewed and 68 patientswere selected to analyze the distribution of their source departments，diagnostic types after referral，and outcomes.Results The first three major source departments were departments of neurology，gastroenterology，and cardiology，accounting for 22.1%，19.1%，and 16.2%，respectively.The first threemajor diagnostic typeswere anxiety，depression，and mental disorder due to physical diseases，accounting for 20.6%，16.2%，and 14.7%，respectively.The outcomes were 48.5%of complete response，and 48.5%of partial response，making up an overall response rate of 97.0%.Conclusion Referrals reflect a change in spectrum of disease and development ofmental health，indicating a close cooperation between psychological department and other departments，further illustrating the importance of establishmentof psychological department in general hospitals.

尽管国家提出所有的三甲医院设立心理科门诊，以解决人民群众日益增长的心理和精神服务的需要，但是综合医院设置精神或者心理科病房，目前仅在我国部分经济发达地区的三级甲等医院设立，虽然近年来有发展的趋势，但是多数设立时间并不长。三甲医院的精神或心理科病房除了日常门诊及病房工作，还要满足其他科室联络会诊的需要，称为会诊-联络精神医学［1］，国内已经有不少综合医院精神科联络会诊的研究报道，但是通过会诊而转入心理或者精神病房治疗的患者目前还没有研究，因此我们通过对我科在院内会诊后收治住院患者进行研究，希望能找出转入心理科治疗的其他科患者的特点。……
