颈部血管超声检查对脑梗死的诊断作用研究

目的 观察评价颈部血管超声检查对脑梗死的诊断作用与意义。方法 该研究方便选取该院住院部在2015年2月—2016年1月期间心血管内科疑似的脑梗死患者作为研究对象，共50例。分别应用经颅多普勒超声、以及颈部血管超声检查。以头颅CT诊断结果为金标准，对比检验结果。结果 应用颈动脉血管超声检查脑梗死病变的灵敏度为97.62%（41/42），特异度为87.50%（7/8）；应用经颅多普勒超声检查脑梗死病变的灵敏度为93.02%（40/43），特异度为74.13%（5/7）；两种检查方法下对脑梗死的诊断结果对比差异无统计学意义（P＞0.05）。结论 颈部血管超声检查对脑梗死诊断有确切价值，可有效掌握检查对象病变血管血流动力学以及斑块形成情况，对脑梗死发病因素判断有参考意义，可为临床治疗提供可靠依据。

［Abstract］Objective To observe and evaluate carotid artery ultrasound for diagnosis of the role and significance of cerebral infarction.Methods In this study，in our hospital unit in February 2015～January 2016 patients with cerebral infarction cardiovascular medicine suspected in January during the study，a total of 50 cases.They were applied transcranial Doppler ultrasound，vascular ultrasound and neck.In head CT diagnosis as the gold standard，comparative test results.Results Carotid artery ultrasound cerebral lesions sensitivity was 97.62%（41/42），specificity was 87.50%（7/8）；transcranial Doppler ultrasound cerebral lesions sensitivity was 93.02%（40/43），specificity was 74.13%（5/7）；diagnostic results under the two methods of cerebral infarction contrast no significant difference was not statistically significant（P＞0.05）.Conclusion Neck vascular ultrasound diagnosis of cerebral infarction exact value，which can effectively control the inspection target lesion hemodynamics and plaque formation，to determine the factors of cerebral infarction have reference value，it can provide a reliable basis for clinical treatment.

我国当前人口老龄化问题已不断加剧，老年群体规模持续扩大，导致心脑血管相关疾病发病率持续上升［1］。脑梗死为常见心脑血管病变之一，对患者的主要危害体现在脑部供血障碍方面，进而可能导致患者脑组织进入缺血、缺氧状态下，后期会导致患者脑组织发生软化、坏死症状，并发神经系统功能缺损问题，严重危及患者生命安全［2-3］。因此，在临床实践中，如何对脑梗死疾病进行早期、有效诊断，已成为心血管内科研究人员的研究重点之一，早期准确诊断脑梗死对改善患者预后也有非常积极的作用［4］。……
