我院肠外营养医嘱点评分析

目的 了解该院住院患者肠外营养处方的临床应用情况，为临床合理应用提供参考。方法 选取2015年1月—2015年9月该院住院患者病历，随机抽取使用肠外营养病历共600份进行点评、记录、归类和回顾性分析。结果 该院肠外营养药物使用基本合理，约35%患者存在糖脂比、热氮比不合理的情况，胰岛素使用量的规范方面有待进一步加强。结论 临床药师应加强与医生的配合，为患者制定科学合理的营养处方，以提高临床营养治疗的安全性和有效性。

［Abstract］Objective To understand the clinical application of parenteral nutrition prescription in our hospital，for the reasonable application.Methods Select the medical records in January 2015-September 2015 in our hospital，random the use of parenteral nutrition records totally 600，comments，records，classified and analyzed retrospectively.Results Parenteral nutrition in our basic rational drug use，there are about 35%appropriateness of species and energy，，appropriateness of the ratios of non—protein calories to nitrogen a，the use of insulin need to be further strengthened.Conclusion Clinical pharmacists should strengthen cooperation with doctors，establish scientific and reasonable nutrition prescription for patients，in order to improve the safety and efficacy of treatment of clinical nutrition.

肠外营养（Parenteral Nutrition，PN）即经静脉补充营养物质，是经静脉途径供应病人所需要的营养要素。肠外营养分为完全肠外营养（TotalParenteralNutrition，TPN）和部分肠外营养（Partial Parenteral Nutrition，PPN）。根据患者的情况，可考虑给予部分或者全部营养支持方式，在临床应用中，TPN应用更普遍。住院患者的营养支持方案及其与临床结局的关系日益引起人们的重视。为确保TPN的合理使用，该院开展了TPN的处方审核和评价，临床药师发挥了重要作用，现将该院临床药师点评的2015年1月—2015年9月共600份住院患者的TPN处方使用情况进行总结分析，以期为规范临床肠外营养药物的合理使用提供参考和依据，现报道如下。……
