大黄联合芒硝外敷早期应用对重症急性胰腺炎肠功能障碍的影响

目的 探讨大黄联合芒硝外敷早期应用对重症急性胰腺炎患者肠功能障碍的影响。方法 方便选取2012年1月—2015年12月该院收治的60例重症急性胰腺炎合并肠功能障碍患者，随机分为对照组和研究组，对照组采取常规治疗，研究组加用大黄联合芒硝腹部外敷，观察肠功能障碍评分、腹内压变化、肠功能恢复时间、住ICU时间、总住院时间等指标。结果 治疗后，研究组的肠功能障碍评分及腹内压下降较对照组明显，差异有统计学意义（P＜0.05）；研究组的肠功能恢复时间、住ICU时间、总住院时间分别为（8.46±3.71）、（10.31±2.56）、（32.45±6.03）d，均较对照组缩短，差异有统计学意义（P＜0.05）。结论 采用大黄联合芒硝外敷早期应用可缓解重症急性胰腺炎患者的肠功能障碍，促进肠功能恢复，改善其预后。

［Abstract］Objective To investigate the effects of rhubarb and mirabilite on intestinal dysfunction in patients with severe acute pancreatitis.Methods In January 2012 to December 2015，60 cases of severe pancreatitis complicated with intestinal dysfunction were selected and divided into the control group and the study group，the control group was treated with conventional therapy，the study group was treated with rhubarb enema andmirabiliteexternal，the intestinal dysfunction score，intraabdominal pressure changes，intestinal function recovery time，live index ICU time，total hospitalization time etc were observated.Results After the treatment，the intestinal dysfunction score and intra-abdominal pressure in the study group decreased significantly compared with the control group，the difference was significant（P＜0.05）；The recovery time of bowel function，ICU time and total length of stay in the study group were（8.46±3.71）d，（10.31±2.56）d，（32.45±6.03）d，were shorter than that of the control group，and the differences were significant（P＜0.05）.Conclusion The combination of rhubarb and mirabilite early application can alleviate intestinal dysfunction in patients with severe acute pancreatitis（SAP）and promote the recovery of intestinal function，improve the prognosis.

肠功能障碍是指“肠实质和（或）功能的损害，导致消化、吸收营养和（或）屏障功能发生严重障碍”［1］。肠道是机体应激的中心器官，是多器官功能障碍发生的动力器官［2］，重症急性胰腺炎患者容易出现肠功能障碍，而肠功能障碍反过来又可加重重症急性胰腺炎病情，因此如何防治肠功能障碍对于扼制重症急性胰腺炎的病情发展，改善其预后，降低其病死率有积极意义。该院ICU2012年1月—2015年12月应用大黄联合芒硝腹部外敷治疗30例重症急性胰腺炎合并肠功能障碍患者，与30例进行常规治疗的重症急性胰腺炎合并肠功能障碍患者相比，其疗效更加显著，现报道如下。……
