定期尿激酶溶栓对永久性双腔留置导管功能及透析充分性的影响

目的 研究定期尿激酶溶栓对永久性双腔留置导管功能及透析充分性的影响。方法 选择2013年1月～2015年1月在我院行维持性血液透析的患者共74例，对照组予以常规肝素封管，观察组在此基础上采用定期尿激酶溶栓治疗。结果 观察组血流量和尿素清除率显著高于对照组。观察组导管通畅率为91.89%，对照组为51.35%，观察组双腔留置导管使用寿命在11个月以上者占97.30%，对照组54.05%，差异有统计学意义（P＜0.05）。结论 在维持性血液透析过程中应用定期尿激酶溶栓，可有效增加导管的通畅性，延长双腔留置导管使用寿命，值提升患者的透析质量。

【Abstract】

Objective To observe the effect of regular urokinase thrombolysis on the function of permanent double lumen catheter and the adequacy of dialysis.

Methods 74 patients with maintenance hemodialysis in our hospital from January 2013 to January 2015 were selected. The control group was treated with routine heparin therapy， and the observation group was treated with regular urokinase thrombolytic therapy. Results The blood flow rate and urea clearance rate of the observation group were significantly higher than that of the control group. The patency rate of the observation group was 91.89%， and the control group was 51.35%. Double lumen catheter used more than 11 months of the observation group was 97.30%， compared with the control group （54.05%） was statistically significant （P＜ 0.05）. Conclusion The use of regular urokinase thrombolysis in the maintenance hemodialysis process can effectively increase the patency of the catheter， prolong the service life of the double lumen catheter， and enhance the quality of hemodialysis patients.

血液透析通路通畅是保证维持性血液透析患者治疗安全、有效的重要前提之一，对透析充分性起着至关重要的作用。临床中许多尿毒症患者由于动脉粥样硬化、血管中层钙化、营养不良等因素给自身动静脉内瘘的建立带来困难，涤纶套的深静脉长期透析导管可以有效补充患者血管通路，永久性双腔留置导管在临床上具有应用价值。本次研究的主要目的是观察研究定期尿激酶溶栓对永久性双腔留置导管功能及透析充分性的影响，选取我院行维持性血液透析患者进行分组对照试验，分析报告如下所述。
