肺炎链球菌感染引起华佛综合征样临床表现10例探究

【摘要】目的 探讨肺炎链球菌感染引起华佛综合征样的临床表现。方法 选取我院2010年1月～2014年12月收治的10例肺炎链球菌感染引起华佛综合征样临床患者，观察患者的临床表现。结果 通过细菌培养与药敏试验，有5例患者的全身皮肤瘀斑迅速扩散呈片状，病情难以控制，经过抢救治疗无效死亡，5例患者经CSF培养出肺炎链球菌，经对症治疗于4周后痊愈出院。结论 肺炎链球菌感染引起的华佛综合征，病原菌为脑膜炎双球菌，病菌感染类型相似，在治疗中要注意鉴别。

Streptococcus Pneumoniae Infection Caused by Sheehan Syndrome Like Clinical Manifestations of 10 Cases of Inquiry

[Abstract]Objective To investigate the clinical manifestations of Sheehan syndrome type caused by Streptococcus pneumoniae infection. Methods 10 cases of patients with this kind of disease admitted in our hospital at the time of January 2010 to December 2014 were selected and their clinical manifestations were observed. Results Through bacterial culture and drug sensitivity test, 5 cases of patients’ systemic skin ecchymosis spread rapidly into sheets, the disease conditions were difficult to control, the patients dead after salvage treatment failure. 5 cases of pediatric patients infected Streptococcus pneumoniae by CSF cultivating, through symptomatic treatment, the patients were discharged 4 weeks later after healing. Conclusion Sheehan syndrome caused by Streptococcus pneumoniae, its pathogenic bacteria is meningococcal meningitis, a bacteria infection of a similar type, attention to the differential in the treatment should be paid.

华佛综合征[1]是由于脑脊髓膜炎引起的暴发型或休克型流脑，患者的临床表现为病发突然，短时间内患者全身出现瘀斑，并且迅速蔓延，同时患者脉象薄弱，血压显著下降，体内循环出现衰竭，导致患者面色苍白，呼吸急促等症状，患者在短时间内如果得不到治疗，将会出现生命危险[2]，我院对10例肺炎链球菌感染引起华佛综合征样临床患者的临床表现进行研究，现将结果报告如下。

1　资料与方法

1.1 一般资料

选取我院2010年1月～2014年12月收治的10例肺炎链球菌感染引起华佛综合征样临床患者，其中男性6例，女性4例，年龄15～28岁，平均年龄（21.56±1.6）岁，病程1～5 d，平均病程（2.1±0.5）d，患者在入院前均有不同程度的发热、面色苍白、呕吐、尿少、神志不清等症状，经入院检查，患者的平均脉搏165次/min，血压均较低，患者的下肢和躯干可以看到大小不等的出血点分散，4例患者深度昏迷，光反应消失，刺激无反应。……
