《肌肉松弛药合理应用的专家共识（2013）》解读

摘 要本文参考相关文献并根据笔者的临床经验解读《肌肉松弛药合理应用的专家共识（2013）》，通过对几个热点问题，包括肌肉松弛药的选择和临床用药方法、在腹腔镜手术中的应用、在危重患者中的应用、过敏反应和新拮抗药等进行讨论，为临床合理应用肌肉松弛药提供参考。

Understanding of the Expert Consensus of the Rational Application of Muscle Relaxants (2013)

ABSTRACTThis paper uses relevant literatures and clinical experience for the interpretation and understanding of the Expert Consensus of the Rational Application of Muscle Relaxants (2013). A couple of hot issues such as the selection and clinical application methods of muscle relaxants, their application in laparoscopic surgery and critically ill patients and allergies, and the new antagonists will be discussed so as to provide a reference for the clinical rational use of muscle relaxants.

《肌肉松弛药合理应用的专家共识》自2009年发布首版以来，受到全国同仁们的欢迎。经二次修订，最新版的《肌肉松弛药合理应用的专家共识（2013）》[1]也已发布，对肌肉松弛药的合理应用更具有临床指导意义。现对其中几个热点问题讨论如下。

1 怎样选择和应用肌肉松弛药

目前临床上使用的肌肉松弛药有琥珀胆碱、维库溴铵、罗库溴铵、阿曲库铵、顺阿曲库铵和米库氯铵等。一般讲，只要没有禁忌证，上述肌肉松弛药均可应用于气管插管和全身麻醉维持。但要做到精确麻醉和合理应用，则应熟悉各肌肉松弛药的药效学和药代动力学。

1.1 气管插管时怎样选用

经静脉诱导施行气管内插管时多选用起效较快的肌肉松弛药或增加肌肉松弛药的剂量以缩短其起效时间。3 ~ 4倍95%有效剂量（95% effective dose, ED95）的琥珀胆碱（1.5 ~ 2.0 mg/kg）的起效时间在1.0 min内；3 倍ED95的罗库溴铵（0.9 mg/kg）的起效时间在1.0 ~ 1.5 min间；4 ~ 6倍ED95的维库溴铵（0.2 ~ 0.3 mg/kg）或顺阿曲库铵（0.2 ~ 0.3 mg/kg）的起效时间可缩短到2 min左右。……
