微型横切口疝囊高位结扎术治疗小儿斜疝的临床疗效分析

【摘要】目的探讨微型横切口疝囊高位结扎术治疗小儿斜疝的临床疗效。方法选择2012年8月～2013年8月在本院住院的小儿斜疝患者80例，随机分为实验组与对照组，各40例。实验组给予微型横切口疝囊高位结扎手术方法治疗，对照组给予传统疝囊结扎手术治疗。两组患者均随访1年，术中及术后观察手术时间，切口长度，术中出血量，住院时间，住院费用，术后并发症，复发率，术中患者的收缩压、心率、呼吸频率及C反应蛋白的变化情况。结果实验组较对照组的手术时间短、切口长度短、术中出血量少、住院时间短、住院费用低、术后出现并发症者例数少、复发率低，具有明显统计学差异(P<0.01)。术中实验组较对照组的收缩压、心率及C反应蛋白升高程度低(P<0.01)，呼吸频率无明显差异(P>0.05)。结论微型横切口疝囊高位结扎术治疗小儿斜疝有手术时间短、损伤小、住院时间短、费用低、并发症少、复发率低及应激小等诸多优点，值得临床推广。

Abstract【】ObjectiveTo evaluate the clinical efficacy of micro-transverse incision hernia sac ligation in treatment of pediatric inguinal hernia.Methods80 patients with pediatric inguinal hernia were randomly divided into experimental and control groups, each contained 40 cases. The experimental group was given transverse incision mini-sac high ligation surgical treatment and control group received traditional sac ligation surgery. All patients participating in the study were followed up for 1 year. The intraoperative and postoperative surgical time, incision length, blood loss, hospital stay, cost of hospitalization, postoperative complications, recurrence rate, systolic blood pressure in patients with surgery, heart rate, respiratory rate and C-reactive protein changes were observed.ResultsCompared with the control group, the experimental group had short operative time, short length of incision, less blood loss, short hospital stay, low hospital costs, less postoperative complications, low recurrence rate (P<0.01). Compared with the control group, the systolic blood pressure, heart rate and low reaction protein C of intraoperative reaction experimental group was increased (P<0.01), with no significant difference in respiratory rate (P>0.05).ConclusionMiniature transverse incision hernia sac ligation treatment of children with inguinal hernia have short operative time, less damage, short hospital stay, low costs, few complications, low recurrence rate and small stress.

小儿斜疝是小儿外科的常见疾病，是由于先天性腹膜鞘状突闭合不良，遗留了囊袋，其通向腹腔，在患者进行使腹压增高的运动时，如哭闹、奔跑及屏气等，肠管和大网膜就能通过此通道被挤到腹股沟内侧皮肤下面，甚至阴囊内，严重影响患者的心理和生活质量[1]。但有研究表明采用传统的手术方法创伤较大，且具有一定的并发症和复发率[2]。……
