前置胎盘患者产后阴道出血的高危因素分析

【摘要】目的探讨前置胎盘患者产后阴道出血的高危因素及治疗方法。方法回顾性分析2011年1月至2013年8月间，在我院住院分娩的76例前置胎盘病例，针对产前各相关因素并对其进行产后出血的药物治疗、局部按摩宫腔沙布填塞压迫及动脉栓塞术治疗等。结果产妇的年龄、流产次数、分娩史、前置胎盘类型、胎盘粘连或植入的前置胎盘孕妇与产后出血有显著相关性(P<0.05)，中央型前置胎盘也与产后出血显著相关(P<0.05)。 结论年龄、流产史、分娩史、前置胎盘类型、胎盘粘连或植入是前置胎盘剖宫产产后出血的高危因素。

Abstract【】ObjectiveTo study the high risk factors and the treatments of postpartum haemorrhage in placenta previa. MethodsThe related risk factors of 76 cases with placenta previa were retrospectively analyzed.ResultsThe relative factors of postpartum haemorrhage in placenta previa were older age，abortion history, history of cesarean section, the type of placenta previa, adhesive placenta or implantable placenta(P<0.05). Central placenta previa were also significantly associated with postpartum hemorrhage(P<0.05). ConclusionThe factors including the age，abortion history, history of cesarean section, the type of placenta previa, adhesive placenta or implantable placenta are the high risk factors of postpartum haemorrhage.

前置胎盘是引起孕晚期及产后阴道出血的主要原因之一，处理不当会危及母子的生命[1-4]。由于医学影像学的发展，目前对产妇前置胎盘可以做到明确诊断。如能够及时掌握产妇前置胎盘形成及产后出血的高危因素，就可以及早地进行干预并积极治疗，以最大限度地保护孕产妇及胎儿的安全。本研究回顾性分析我院2011年1月至2013年12月收治的76例前置胎盘孕妇的病史资料，旨在探讨前置胎盘剖宫产术大出血的危险因素,现报告如下。

1资料与方法

1.1一般资料符合《妇产科学》诊断标准[5]，明确诊断为前置胎盘的产妇76例，年龄24～42岁，平均年龄(26.9±4.7)岁，22例曾经有流产史，11例为初次分娩，35例为二次分娩，二次以上分娩8例，入院时孕周30～37周，平均(32.95±4.76)周。76例前置胎盘中，边缘性前置胎盘35例，中央性前置胎盘21例，部分性前置胎盘30例。……
