儿科住院部头孢曲松钠剂量使用情况分析

摘要：目的了解我院儿科住院部头孢曲松钠剂量使用情况，为安全合理用药提供参考。方法 对我院2012年8月～2014年7月的儿科住院部210例使用头孢曲松钠的病历，按性别，剂量，合并用药，细菌培养，肝肾功，血常规，转归等信息进行统计分析。结果使用剂量在80 mg·kg-1以上的患儿有58例，占27.62%，其中包括新生儿。结论头孢曲松钠的使用存在剂量偏高的现象，应引起医学工作者的关注。

Analysis of the dose usage situation of ceftriaxone sodium in inpatients in the department of pediatrics

Abstract:ObjectiveTo investigate the dose usage situation of ceftriaxone sodium in inpatients in the department of pediatrics of our hospital，and provide reference for rational drug use.Method210 cases treated with ceftriaxone sodium from August 2012 to July 2014 of inpatients in the department of pediatrics，according to gender，doses， the drug combination， bacteria culture， liver and kidney function， blood routine, and outcome informations, were statistically analyzed.ResultDose above 80 mg·kg-1 was 58 cases，accounted for 27.62%，including the newborn.ConclusionThe use of ceftriaxone sodium exists the phenomenon of over dose.Medical workers should pay attention to it.

头孢曲松钠为半合成的第三代头孢菌素，临床上广泛应用于敏感菌所致的感染。其不良反应有变态反应、胃肠道反应、肝肾毒性、造血系统毒性及中枢神经系统毒性等[1-2]，不良反应的发生与治疗的剂量和疗程有关。本文统计分析了我院儿科住院部的210例病例，目的在于了解头孢曲松钠超剂量的使用情况，为合理应用提供依据。

1资料与方法

1.1资料本研究抽取2012年8月～2014年7月我院儿科住院病人开具头孢曲松钠的病历210例。

1.2剂量拟定依据头孢曲松钠说明书的剂量,成人及12岁以上儿童通常剂量是1～2 g，每日1次，危重病例或由中度敏感菌引起的感染，剂量可增4 g，每日1次。 新生儿(14 d以下)每日剂量为按体质量20～50 mg·kg-1不超过50 mg·kg-1，婴儿及儿童(15 d～12岁)每日剂量按体质量20～80 mg·kg-1。英国国家儿童处方集规定：新生儿静脉滴注每天20～50 mg·kg-1(最大剂量每天50 mg·kg-1)。……
