乌灵胶囊治疗卒中后焦虑及改善神经功能疗效的系统评价

摘要:目的系统评价乌灵胶囊对卒中后焦虑的疗效及改善神经功能和日常生活活动能力的疗效。方法计算机检索CNKI、CBM、WanFangData,检索时限为建库至2014年12月。采用RevMan5.2软件进行Meta分析。结果最终纳入12个随机对照试验,共计1 020例患者。Meta分析结果显示:与对照组相比,治疗组的焦虑评分改善优于对照组,差异有统计学意义[MD= -2.86, 95% CI(-4.88,-0.84),P = 0.006]。治疗组美国国立卫生研究院卒中量表(NIHSS)评分下降值高于对照组,差异有统计学意义[MD= -2.57,95% CI(-3.74,-1.40),P<0.000 1]。与对照组相比,治疗组随访期末的BI指数高于对照组,差异有统计学意义[MD= 7.19,95% CI(4.37,10.00),P<0.000 01]。结论乌灵胶囊可改善卒中后焦虑、神经功能和日常生活活动能力。 2.1 文献检索结果初检出相关文献72篇,经阅读文题、以及阅读全文,排除不符合纳入标准的、不能提取数据的文献,最后纳入12篇文献,共计1 020例患者。

Abstract:Objective To systematically review the effectiveness of Wuling capsule for patients with anxiety and improvement of nerve function and daily life activities after stroke.Methods Such databases as Chinese National Knowledge Infrastructure(CNKI),China Biomedical Database(CBM)and WanFang Data were electronically researched to collect the randomized controlled trials(RCTs)about Wuling capsule treating patients with post stroke anxiety from the datesof their establishment to December 2014.Met a analysis was performed using RevMan 5.2 softword.Results Twelves RCTs involving 1020 cases were included.The results of met aanalysis showed that compared to the control group,Wuling capsule group was more effective in decreasing anxiety scores(MD= -2.86,95% CI 4.88 to-0.84,P = 0.006),NIH Stroke Scale(NIHSS)scores(MD= -2.57,95% CI -3.74 to -1.40,P <0.0001).The BI scores in Wuling capsule group were higher than that in control group(MD= 7.19,95% CI 4.37 to 10.00,P<0.000 01).Conclusion

脑卒中后焦虑抑郁是脑卒中后最常见的精神障碍并发症,主要表现为情绪焦虑抑郁、持续性心慌、睡眠障碍等,已成为影响患者功能恢复和卒中复发的独立危险因素。目前,已有研究证实乌灵胶囊可有效治疗卒中后抑郁(pos t stroke depression,PSD),改善卒中后抑郁症状且其不良反应少。但对于是否能治疗卒中后焦虑、改善神经功能、提高日常生活能力的研究甚少。本研究旨在系统评价乌灵胶囊治疗卒中后焦虑的疗效和对神经功能、日常生活活动能力的改善情况,为卒中后焦虑的进一步治疗及研究提供依据。
