应用超声弹性成像技术对高血压合并糖尿病患者血管弹性变化的诊断效果

[摘要] 目的 探讨对高血压合并糖尿病患者应用超声弹性成像技术诊断血管弹性变化的效果。方法 观察组随机选取2013年3月—2014年10月该院收治的高血压合并糖尿病患者70例,另选取60例健康体检者作为对照组,对所有患者应用超声测量动脉内中膜厚度,再利用超声弹性成像技术对所有动脉测量弹性系数比值(B/A值)测量,对比两组患者动脉内中膜厚度及B/A值。结果 与正常对照组相比,观察组患者颈动脉、胫前动脉、股动脉、足背动脉、腘动脉内中层厚度以及B/A比值明显增高,差异有统计学意义(P<0.05);观察组腘动脉、胫前动脉、足背动脉与桡动脉、股动脉之间B/A值差异有统计学意义(P<0.05)。 结论 超声弹性成像技术对高血压合并糖尿病患者血管弹性变化的诊断效果良好。

Diagnostic Effect of the Ultrasound Elastography Application on the Change of Blood Vessels Elasticity in Patients Consolidated with Hypertension and Diabetes

[Abstract] Objective To investigate the changes of vascular elasticity in patients with hypertension complicated with diabetes mellitus by using the ultrasonic elasticity imaging technique. Methods 70 cases of patients consolidated with hypertension and diabetes treated in our hospital between March 2013 and October 2014 were selected as the observation group, while another 60 healthy subjects were selected as the control group. We measured the thickness of the membrane with ultrasound and applied the ultrasound elastography to measure all the arterial elasticity ratio (B / A value).Then to compare the arterial thickness and B / A value in the two groups of patients. Results Compared with the control group, the membranes of carotid, anterior tibial artery, femoral artery, dorsal artery, popliteal artery of the patients and B / A value in the observation group were significantly higher than those of the patients in control group, the difference was statistically significant(P <0.05); the difference in the values of popliteal artery, anterior tibial artery, dorsal artery and radial artery, and femoral artery in the observation group were statistical significant (P <0.05). Conclusion The ultrasound elastography has a good effect on the diagnosis of change of blood vessels elasticity in patients consolidated with hypertension and diabetes.

高血压和糖尿病是引起心血管疾病的最主要危险因素之一,也是致患者死亡始发因素。高血压及糖尿病主要危害全身大小动脉,致动脉壁结构以及功能的改变,其主要特征表现为动脉弹性下降以及随后出现血管粥样硬化及斑块形成[1]。其中动脉弹性减低是动脉硬化发生的始动环节,并且是早期反映患者心血管疾病的最重要指标,因此积极的对动脉弹性功能的监测对心血管疾病的预防具有重要意义[2]。目前,临床上对动脉弹性功能检测最常用脉搏传导速度、脉压来反映动脉弹性,但是由于脉压增大往往发生在血管弹性减低晚期,脉搏传导速度敏感性差,并不能作为动脉弹性功能唯一的评估指标[3]。超声弹性成像技术是近年来发展的新的超声检查技术,其能够通过对血管组织施加一个动态激励,利用血管壁移动来评价血管弹性能力[4]。……
