[摘要] 目的探讨牙周-正畸联合治疗成人牙周炎患者的方法及临床疗效。 方法 回顾性分析2008年3月～2010年4月我院经牙周-正畸联合治疗的37例成人牙周炎患者临床资料，并观察正畸治疗后23～48个月远期疗效。 结果 正畸治疗后37例患者咬合关系正常，前牙覆覆盖正常，牙齿排列整齐，邻接关系恢复，尖窝接触关系良好，牙齿松动明显好转，牙龈探诊未见出血，X线片检查显示牙槽骨吸收停止或有增生影像。随访23～48个月，37例患者基本做到半年复查一次，其中无复发29例（78.4%），轻度复发8例（21.6%），无明显复发者。 结论 牙周-正畸联合治疗牙周炎能有效控制炎症，消除咬合创伤，排齐牙列，利于牙周组织痊愈及患牙固定，恢复患牙咀嚼和美观功能，保持长期稳定的治疗效果。

Observation of the effect of periodontal-orthodontic treatment on the adult periodontitis

[Abstract] Objective To explore the methods and clinical effect of periodontal-orthodontic treatment on the patients with periodontitis. Methods A retrospective analysis was performed on the clinical data of 37 adult patients with periodontitis,who treated by periodontal-orthodontic treatment in our hospital from Match 2008 to April 2010 and observed their long-term follow-up result for 23 to 48 months after the treatment. Results the 37 patients'occluding relation,overlap and overbite of anterior teeth was normal,their teeth arranged orderly, adjacency relation was recovered,canine fossa contact relationship was good and gomphiasis situation turned better obviously. Examined the gingivas of the patients found there was no bleeding,examined by X-ray found their alveolar resorption stopped,some of the patients showed the hyperplasia images. Followed-up visit for 23 to 48 months,the 37 patients reviewed once half a year basically,29 cases of them no relapse (78.4%),8 cases of them relapse mildly (21.6%),there was no patient relapse obviously. Conclusion periodontal-orthodontic treatment to treat the periodontitis can control the inflammation effectively, eliminate the occlusal traumatism,arrange the denture,benefit to periodontium recovery and offending teeth immobilization,recover the chew and aesthetic function of the offending teeth,and maintain their secular stability and therapeutic effect.

成人牙周炎常造成继发性创伤，对牙周组织损害性较大，进一步加重牙周症状，在牙周炎症消除后通过正畸治疗可消除创伤，改善咬合关系，排齐牙列，利于进一步牙周维护，提高治疗效果，经过牙周系统治疗消除炎症，即使已发生附着水平下降和牙槽骨吸收也可矫治[1]。2008年3月～2010年4月我院采用牙周-正畸联合治疗成人牙周炎患者37例，收效良好，报道如下。

1 资料与方法

1.1 一般资料

37例患者中男14例，女23例，年龄18～34岁，平均（23.4±2.2）岁，患者均有牙松动、伸长、移位、牙间隙增宽、倾斜等表现，其中深覆11例，上前牙前突、散在间隙14例，前牙反、下前牙扇形散开12例，牙周袋探针深度3～5mm，附着丧失＜5mm，牙齿松动度Ⅰ°～III°，X线片检查显示患牙牙槽骨不同程度吸收。……
