肌电生物反馈联合抗阻训练对老年骨关节炎患者膝关节功能的影响

【摘要】 目的:探讨肌电生物反馈联合抗阻训练对老年骨关节炎患者膝关节功能的影响。方法:选取2020年1月—2023年1月九江市第一人民医院收治的老年骨关节炎患者102例,随机分为两组,其中对照组(n=51)采用抗阻训练治疗,观察组(n=51)在对照组基础上联合肌电生物反馈治疗,比较两组临床疗效、疼痛评分、膝关节功能及等速肌力参数水平。结果:观察组总有效率为90.20%,高于对照组的74.51%,差异有统计学意义(P<0.05)。干预8周,观察组视觉模拟评分法(VAS)评分、改良版西安大略和麦克马斯特大学骨关节炎指数(WOMAC)评分均低于对照组,Lysholm膝关节评分量表(LKSS)评分高于对照组,差异均有统计学意义(P<0.05)。干预8周,观察组等速肌力参数峰力矩(PT)、总做功量(TW)、疲劳指数(WF)水平均高于对照组,差异均有统计学意义(P<0.05)。结论:老年骨关节炎患者采用肌电生物反馈联合抗阻训练疗效显著,能有效减轻患膝疼痛程度,改善膝关节屈伸肌力及活动功能。

[Abstract] Objective: To investigate the effect of myoelectric biofeedback combined with resistance training on knee joint function in elderly patients with osteoarthritis. Method: A total of 102 elderly patients with osteoarthritis admitted to Jiujiang NO.1 People's Hospital from January 2020 to January 2023 were selected and randomly divided into two groups. The control group (n=51) received resistance training treatment, and the observation group (n=51) combined with myoelectric biofeedback treatment. The clinical efficacy, pain score, knee joint function and isokinetic muscle strength parameters of the two groups were compared. Result: The total effective rate of observation group was 90.20%, which was higher than 74.51% of control group, the difference was statistically significant (P<0.05). 8 weeks of intervention, the visual analogue scale (VAS) score and the modified Western Ontario and McMaster University osteoarthritis index (WOMAC) score in the observation group were lower than those in the control group, and the Lysholm knee joint score scale (LKSS) score was higher than that in the control group, the differences were statistically significant (P<0.05). 8 weeks of intervention, isokinetic muscle force parameters peak moment (PT), total work done (TW) and work fatigue (WF) in observation group were higher than those in control group, the differences were statistically significant (P<0.05). Conclusion: The effect of myoelectric biofeedback combined with resistance training in elderly patients with osteoarthritis is significant, which can effectively reduce the degree of knee pain, improve knee flexion and extension muscle strength and activity function.

骨关节炎是临床常见慢性退行性关节疾病,好发于中老年人群,可导致患者出现不同程度的关节活动受限、疼痛,甚至畸形。目前,临床治疗骨关节炎尚无特效药物,运动疗法、物理疗法被认为是骨关节炎关节功能改善的有效治疗措施,其中抗阻训练被证实能在减轻患者关节疼痛、增强肌肉力量和改善关节功能方面发挥一定的作用[1-2]。但也有研究报道,部分患者经抗阻训练干预后仍存在关节运动功能恢复缓慢,甚至关节疼痛反复发作等问题,故临床建议采用联合治疗方案[3]。随着物理因子治疗技术的发展,肌电生物反馈在肌肉骨骼系统疾病康复治疗领域应用越来越广泛,其可通过反复刺激神经、肌肉,实现肌电和神经肌肉电刺激的有效结合,进而促进神经、肌肉功能恢复[4-5]。但目前,临床关于肌电生物反馈联合抗阻训练应用于骨性关节病变方面的研究报道较少,缺乏有效参考依据。……
