点阵激光治疗白癜风进展

[摘要]白癜风的治疗方法多样,主要有光疗、药物治疗以及外科疗法等。近年来,由于点阵激光具有治愈率高,安全性好,副作用小等优点,广泛应用于白癜风的临床治疗。为缩短治疗时间,提高患者依从性,许多学者提出联合疗法,如:联合钙调磷酸酶抑制剂、糖皮质激素、外科治疗、中医中药及黑光,甚至综合疗法。本文就点阵激光治疗白癜风的进展作一概述。

Update on Fractional Laser Treatment of Vitiligo

Abstract: The treatment of vitiligo is various, including phototherapy, drug therapy and surgical therapy.In recent years, fractional laser has been widely used in the clinical treatment of vitiligo due to its advantages of high cure rate, good security and slight side effects. In order to shorten the treatment time and improve the compliance of patients, many scholars have proposed combination therapy, such as combined calcineurin inhibitor,glucocorticoid, surgical treatment, Traditional Chinese Medicine, ultraviolet, and even comprehensive therapy. In this paper, the progress of fractional laser treatment of vitiligo is summarized.

白癜风是一种常见的后天性色素脱失性皮肤病,全球发病率约为1%[1]。其发病机制尚不明确,可能与遗传、免疫、氧化应激、黑素细胞自身破坏等多种因素相关[2-5]。因其影响美观与社交,往往对患者造成严重的心理负担和精神压力[6]。白癜风的治疗方法多样,主要有光療、药物治疗以及外科疗法等。近年来,由于点阵激光具有治愈率高,安全性好,副作用小等优点,被广泛应用于白癜风的临床治疗。本文就点阵激光治疗白癜风的进展作一概述,以期为临床应用提供参考。

1  点阵激光的治疗原理与作用机制

点阵激光是在传统超脉冲激光的基础上添加了计算机图形发生器,使其治疗深度与范围可控。其基本原理是点阵式光热作用(Fractional photothermolysis,FP),其用于白癜风的治疗是因为[7]:①热作用可增大黑素细胞胞体,增加树突数量,提升酪氨酸酶活性,促进黑素合成;②热作用导致胶原束变性,从而使皮损区域组织收缩,皮损变小;③可促进皮损区分泌细胞因子和生长因子,调节黑素生成,促进黑素细胞增殖以及迁移。

2  单一疗法

Hu等[8]对25例白癜风患者给予每周1次点阵CO2激光治疗,经4~8次治疗后,6例患者痊愈,显效率72.0%(18/25),好转率92.0%(23/25),且用ELISA法检测患者前后细胞因子变化发现,治疗后IL-4、IL-10、IL-17、IL-23水平较治疗前明显降低(P<0.05),IL-2、IFN-γ水平亦降低,但差异无统计学意义(P>0.05)。

3  联合应用

临床上,由于单一疗法治疗效果有时不甚理想,许多学者提出了联合疗法,既往的系统性回顾与Meta分析表明[9-11],点阵CO2激光是各种白癜风的有效辅助治疗方案,可作为常规治疗的补充。主要的联合应用方案如下:

3.1 点阵激光联合钙调磷酸酶抑制剂:他克莫司属于新型免疫调节剂,可以抑制T细胞活性,从而保护黑素细胞,但如果单纯使用药物涂抹,往往达不到理想效果。Chen等[7]将45例白癜风患者随机分为治疗组和对照组,两组均外用0.1%他克莫司乳膏,治疗组每月接受1次点阵CO2激光治疗,6个月后治疗组患者皮损愈合率、有效率均明显高于对照组,差异有统计学意义(P<0.05)。而刘晶等[12]采用非剥脱1 550nm Er:YAG激光联合他克莫司疗效不佳,可能是由于1 550nm Er:YAG激光剥脱的深度并未完全达到基底层。

3.2 点阵激光联合糖皮质激素:梅册芳等[13]将60例稳定期白癜风患者分为两组,治疗组予点阵CO2激光联合丙酸氟替卡松乳膏治疗;对照组予308nm准分子光联合丙酸氟替卡松乳膏治疗。12周后治疗组有效率85.75%,高于对照组有效率70.83%(P<0.05)。分析其原因可能点阵CO2激光可在皮肤构建药物传输通道,有利于药物的传输及渗透。周星等[14]证明点阵激光可提高复方倍他米松治疗白癜风的疗效。

3.3 点阵激光联合外科治疗:自体表皮移植治疗白癜风疗效肯定,但皮损区处理时其深度与边界较难控制,且处理眼周、口周等特殊部位操作受限。Lagrange等[15]对比使用2 940nm Er:YAG点阵激光或微针磨削皮损区后再行自体表皮细胞悬液移植术在治疗白癜风疗效上的差异,发现激光磨削组复色效果优于微针组。Silpa-Archa等[16]也证实点阵CO2激光联合黑素细胞-角质形成细胞移植(Melanocyte-keratinocyte transplantation procedure,MKTP)是一种疗效佳,复色率高,操作简单的治疗方式。然而,一项对节段性白癜风和斑驳病进行的前瞻性随机对照试验显示[17],全表面磨削术优于点阵CO2激光,这可能是由于使用自体表皮细胞悬液时,黑素细胞只占移植细胞的一小部分,且穿透表皮的细胞数量有限,不足以产生复色。

3.4 点阵激光联合中医中药:随着中医中药的蓬勃发展,一些学者[18-20]将点阵CO2激光与中药,如:复方卡力孜然酊、复方槐花补骨脂酊、复方甘草酸苷片联用治疗白癜风,疗效确切。火针作为中医外治疗法的重要组成部分,既有针的机械刺激,又有火的温热刺激,温经通络、活血化瘀,不少学者将其应用于白癜风的治疗。刘景卫等[21]对点阵CO2激光联合龙胆泻肝汤治疗白癜风进行临床研究,治疗组采用点阵CO2激光联合龙胆泻肝汤,对照组采用火针联合龙胆泻肝汤,3个疗程结束后治疗组总有效率(83.3%)显著高于对照组(39.3%),这可能与点阵CO2激光较火针对皮损的刺激深浅更精准、强度更可靠及分布更均匀有关。

3.5 点阵激光联合黑光:黑光治疗白癜风疗效显著,但是单纯运用黑光需要反复多次治疗,许多患者因为无法长期坚持而影响疗效。不少学者[22-23]将点阵CO2激光与黑光联用,疗效显著,安全性高,缩短了治疗时间,给患者更多信心,有望提高患者的依从性。Doghaim等[24]对32例稳定性非节段性白癜风患者的对称性皮损分别予点阵CO2激光联合NB-UVB与NB-UVB治疗,联合侧不仅在复色率方面显著高于NB-UVB侧,且联合侧治疗结束后,HE染色显示明显的黑色素沉着,Melan-A有明显表达,而NB-UVB側仅有微弱的黑色素沉着,Melan-A有轻至中度表达。然而Chang等[25]对2019年7月之前所有CO2点阵激光联合NB-UVB的文献统计分析,在排除了使用NB-UVB以外光疗的研究和使用非平行附加药物的试验后得出结论,CO2点阵激光并不能提高NB-UVB治疗的复色率。Esme等[26]和El-Zawahry等[27]也在试验中证明CO2点阵激光联合NB-UVB治疗白癜风与单纯NB-UVB治疗相比无明显优势。针对这种争议的情况,Kim等[28]从不同的数据库中检索了截至2020年1月前的数据,在剔除异质性高的文章后得出结论,点阵CO2激光联合NB-UVB治疗非节段性白癜风比单纯NB-UVB更有效;研究还发现,点阵CO2激光治疗后数天进行NB-UVB照射比立即开始NB-UVB照射更有效。

3.6 点阵激光联合其他疗法:PRP(Platelet rich plasma)又名富血小板生长因子(Platelet-rich growth factors,GFs),是一种高浓度血小板的血浆制品,临床中通过将自体PRP注射到病灶处来发挥作用[29]。Abdelghani等[30]将80例稳定期非节段型白癜风患者随机分为四组,即CO2点阵激光组、PRP组、点阵CO2激光联合PRP组以及点阵CO2激光联合NB-UVB组,比较组间疗效。结果表明,点阵CO2激光联合PRP组复色效果最好,复色率>50%的占60%,复色率>75%的占40%;其次是点阵CO2激光联合NB-UVB组,复色率>50%的占25%,复色率>75%的占5%,这与Kadry等[31]研究结果一致。Garg等[32]将2 940nm Er:YAG点阵激光与PRP联用,也取得了令人满意的效果。同样是使用2 940nm Er:YAG点阵激光,Abdelwahab等[30]比较2 940nm Er:YAG点阵激光联合5-FU与5-FU单药治疗非节段性白癜风的疗效差异,4~6周为1个疗程,3个疗程后,试验组轻度(<25%)复色占73.3%,中度(50%~75%)复色占10%,远高于对照组5%的复色率。

4  综合疗法

随着人们对白癜风认识的深入,临床上治疗白癜风的方法也越来越多,许多学者提出了各式各样的综合疗法,丰富了白癜风的治疗方案。NB-UVB、308nm准分子光、药物穿透皮肤深度有限,不同部位表皮厚度不同,手背表皮厚度可达1~2mm,这可能是导致其治疗肢端白癜风疗效差的原因。而点阵CO2激光能磨削较厚的表皮,且能在皮肤构建药物传输通道,有利于药物的传输及渗透,与药物三者联合使用有更好的疗效。Vachiramon等[33]对非节段型手部白癜风患者分别予点阵CO2激光联合NB-UVB及0.05%丙酸氯倍他索,NB-UVB联合0.05%丙酸氯倍他索,结果表明前者23.1%的患者复色率大于50%,而后者仅3.9%的患者复色率大于50%,两种光联合激素治疗手部白癜风患者疗效更好。在一项多中心研究中,Liu等[34]对126例稳定期肢端白癜风患者的对称性皮损分别给予点阵CO2激光联合NB-UVB及复方倍他米松溶液和点阵CO2激光联合NB-UVB及倍他米松乳膏,试验侧总有效率为51.6%,显著高于对照侧的总有效率35.8%。这表明点阵CO2激光对表皮的磨削使复方倍他米松溶液相较于倍他米松乳膏更易于深入渗透。而Wen等[35]对21例多发性、局限性、难治性、非节段性白癜风给予308nm准分子光、点阵CO2激光与他克莫司软膏的综合治疗,其疗效并不优于308nm准分子光联合他克莫司软膏,其治疗失败可能与他克莫司软膏未充分渗透点阵CO2激光在皮肤上形成的孔道有关。

對于点阵激光治疗白癜风来说,表皮剥脱是白斑复原的重要原因,非剥脱点阵激光无法有效去除表皮,可能导致治疗效果不佳。唐娟等[36]对80例传统治疗无效的稳定期非节段型白癜风患者分别给予不同的治疗方案:方案1:剥脱性点阵CO2激光联合卤米松和NB-UVB治疗;方案2:NB-UVB治疗;方案3:非剥脱点阵CO2激光联合卤米松和NB-UVB治疗,结果表明剥脱性点阵CO2激光联合卤米松和NB-UVB治疗稳定期非节段型白癜风患者临床疗效可靠,较非剥脱性点阵激光联合治疗临床效果更优。这与Yuan等[37]对比剥脱性CO2点阵激光与非剥脱1 565nm点阵激光联合NB-UVB、倍他米松溶液的结论一致。Yan等[38]报道了1 800mJ/P高能量的2 940nm Er:YAG点阵激光联合NB-UVB治疗后,外涂复方倍他米松注射液,60%的患者白斑复色率大于50%,1 200mJ中能量的2 940nm Er:YAG点阵激光辅助治疗后,36%的患者白斑复色率大于50%,两治疗组疗效显著高于仅接受NB-UVB治疗的对照组,然而,600mJ低能量的2 940nm Er:YAG点阵激光辅助治疗与单纯NB-UVB治疗相比没有明显差异。

5 小结

综上,点阵激光作为一种新型光疗技术,已成为白癜风治疗的有效手段。单独应用点阵激光治疗白癜风仍有一定局限性,联合应用或综合疗法可以提高疗效,缩短疗程,减少不良反应。由于点阵激光技术发展时间短,临床研究样本量较小,仍需对其理论研究、临床应用及不良反应进行深入探讨,从而优化治疗方案。
