【摘要】 目的：研究應用ProTaper机用镍钛锉对后牙根管治疗的效果。方法：选取2017年10月-

2018年10月于本院进行后牙根管治疗的208例患者，随机分为对照组和试验组，每组104例。对照组采用常规的不锈钢根管K锉预备根管，试验组采用ProTaper机用镍钛锉进行根管预备，比较两组的根管预备后的疼痛、预备的时间、根管阻塞情况、根管成形的效果、根管充填的效果和根管治疗的成功率。结果：试验组疼痛的发生率较低（P<0.05），根管的预备时间明显缩短（P<0.05），根管预备的阻塞率降低（P<0.05），根管预备的成形效果明显提高（P<0.05），根管充填的效果明显提高（P<0.05），远期根管治疗预后较好（P<0.05）。结论：ProTaper机用镍钛锉在后牙的根管治疗中应用效果好，能明显减少预备的时间，根管预备阻塞率低，根管成形效果好，根管充填远期成功率高，缓解了根管预备后的疼痛，临床效果较好。

[Abstract] Objective： To investigate the clinical effect of the machine-made ProTaper nickel-titanium file and stain-less steel k file in the root canal treatment of posterior teeth. Method： A total of 208 patients with 208 teeth were selected from January to October 2017 to October 2018 for root canal treatment of posterior teeth in our hospital. The patients were randomly divided into nickel-titanium file group （104 cases， 104 teeth） and stainless steel group （104 cases， 104 teeth） according to the order of their visit. They were treated with ProTaper nickel-titanium file and stainless steel K file respectively. The root canal treatment effects of the two groups were compared on pain rates， root canal preparation time， root canal occlusion， forming effects， root canal filling effect and root canal long-term success rate. Result： The pain rates after operation in the nickel-titanium file group were significantly lower than those in the stainless steel group， the differences were statistically significant （P<0.05）. The root canal preparation time of nickel-titanium file group were significantly lower than those of stainless steel group （P<0.05）. The root canal occlusion of nickel-titanium file group were significantly lower than those of stainless steel group （P<0.05）. Good rate of root canal preparation shape of nickel-titanium file group was significantly higher than that of stainless steel group， the differences were statistically significant （P<0.05）. The filling rate of the nickel-titanium file group was obviously higher than that of the stainless steel group， the differences were statistically significant （P<0.05）. The root canal long-term success rate of the nickel-titanium file group was obviously higher than that of the stainless steel group， the differences were statistically significant （P<0.05）. Conclusion： The machine-made ProTaper nickel-titanium file has good clinical effect in posterior teeth root canal treatment， shortens root canal preparation and reduced root canal occlusion， increases forming effects， improves the good long term success rate and reduces the incidence of postoperative pain， is safe and reliable， and have good clinical effects.

根管治疗术是目前治疗牙髓病及根尖周病的主要手段[1]，根管治疗是通过尽量完善的三维预备、适当的消毒来彻底清除根管内的感染物质，去除根管内有害物质对根尖周组织的不良刺激，并通过严密的根管充填，从而防止根尖周病变的发生或促进根尖周病变愈合[2]。众所周知，成功的根管治疗主要由三方面决定—认识根管、充分制备、严密充填。根管充填是根管治疗的最终步骤，其目的在于通过严密封闭根管系统，消除病原刺激物从口腔和根尖周组织侵入根管系统的途径，隔离残留于根管中的病原体，为促进根尖周组织病变愈合、预防根管系统再感染创造有利的生物学环境。严密的根管充填是根管治疗术成功的关键因素，而根管的预备是严密根管充填的前提条件，根管预备的充分，根管内感染残留的病原体清理的越干净，后续的根管消毒越彻底，根管充填的远期效果越好。本研究应用ProTaper机用镍钛锉治疗后牙根管，取得不错效果，现报道如下。……
