[摘要] 本院互联网医院是针对本院复诊、病情稳定的慢性病患者利用信息化手段借助互联网平台探索出一种将慢性病电子处方流转至社会药店的配药服务模式。用户在我院微信公众号平台上进入我院互联网医院程序，通过多种模式申请慢性病复诊续方服务，实体医院医生审核并开具处方，用户自主选择就近药店，药店药师审方通过后用户付费取药。这一服务的应用，真正实现了“让数据多跑路，患者少跑路”的愿景，改善了慢性病患者“因药就医”的就诊体验，并在实际应用中收获了明显效果。

Application and experience of continuous prescription service for chronic disease in our internet hospital

[Abstract] Our Internet hospital has explored a dispensing service model for transferring chronic disease electronic prescriptions to social pharmacies for chronic diseases patients with referral in our hospital and in stable conditions by using information technology and Internet platform. Users enter our hospitals Internet hospital program on our hospitals WeChat official account platform， and apply for referral and continuous prescription service of chronic disease through multiple modes. The physical hospital doctor reviews and writes prescriptions. The user chooses the nearest pharmacy， and pays for it after the pharmacy pharmacist reviews the prescription. The application of this service has truly realized the vision of "let data run more， patients run less". It improves the experience of "seeing a doctor for medicine" for patients with chronic diseases， and have achieved significant results in practical applications.

隨着我国逐渐进入老龄化社会，慢性病群体日益庞大[1-2]。慢性病患者多病情稳定，因长期服药需到医院门诊定期配药，一定程度上浪费了医院有限的医疗资源，诸多流程也费时费力。慢性病药品约占医院总药品使用金额的60%[3]，高额的药品费用无形中增加了医院费用及药占比的压力。公立医院药品零加成、药占比限制、医保控费等政策成为处方外流的重要推动力。随着“互联网+医疗”的飞速发展，我院通过互联网医院助力电子处方流转，将线下就诊过的、病情稳定的慢性病患者引导到线上来，患者在线上申请后得到电子处方，选择家附近药店取处方药，可免掉去医院排队等待的麻烦，大大缩短了患者就诊时间，改善了就诊体验，破解了“因药就医”的难题。……
