【摘要】 目的：探究血糖變异性对糖尿病视网膜病变患者眼血流参数的影响。方法：选取2018年10月-2020年6月的120例糖尿病视网膜病变患者为观察组，同时期120名健康者为对照组。比较两组各级动脉（眼动脉、视网膜动脉及睫状后动脉）血流参数（Vs、Vd、PI及RI），比较观察组不同空腹血糖变异系数患者上述眼血流参数，采用Spearman秩相关分析上述眼血流参数与空腹血糖变异系数的关系。结果：观察组眼动脉、视网膜动脉及睫状后动脉Vs、Vd均显著低于对照组，PI、RI均显著高于对照组（P<0.001）。观察组不同空腹血糖变异系数患者上述眼血流参数比较，差异均有统计学意义（P<0.001）。Spearman秩相关分析显示，眼动脉、视网膜动脉及睫状后动脉Vs、Vd与空腹血糖变异系数均呈负相关，PI、RI与空腹血糖变异系数均呈正相关（P<0.05）。结论：血糖变异性对糖尿病视网膜病变患者眼血流参数的影响较大，因此应重视对糖尿病视网膜病变患者上述方面的监测与调控。

[Abstract] Objective： To investigate the influence of blood glucose variability for ocular blood flow parameters of patients with diabetic retinopathy. Method： A total of 120 patients with diabetic retinopathy from October 2018 to June 2020 were chosen as the observation group， 120 healthy persons at the same time were chosen as the control group. Then the arteries at different stages （ophthalmic artery， retinal artery and posterior ciliary artery） blood flow parameters （Vs， Vd， PI and RI） of two groups were compared， and those ocular blood flow parameters of the observation group with different variation coefficient of fasting blood glucose were compared， then the correlation between those ocular blood flow parameters and variation coefficient of fasting blood glucose were analyzed with Spearman rank correlation analysis. Result： The Vs and Vd of ophthalmic artery， retinal artery and posterior ciliary artery in the observation group were significantly lower than those of the control group， the PI and RI were significantly higher than those of the control group （P<0.001）. And those ocular blood flow parameters of the observation group with different variation coefficient of fasting blood glucose were compared， there were statistically significant differences （P<0.001）. Spearman rank correlation analysis showed the Vs and Vd of ophthalmic artery， retinal artery and posterior ciliary artery were negatively correlated with variation coefficient of fasting blood glucose， the PI and RI were positively correlated with variation coefficient of fasting blood glucose （P<0.05）. Conclusion： The influence of blood glucose variability for ocular blood flow parameters of patients with diabetic retinopathy are great， so those items of patients with diabetic retinopathy should be paid to more monitoring and control.

糖尿病视网膜病变是糖尿病常见并发症，其对患者的眼部损伤突出，可表现出视力障碍等情况，是临床常见致盲性疾病，因此临床对糖尿病视网膜病变的防控重视程度较高[1]。另外，在糖尿病视网膜病变相关的研究中，眼部血流是研究较多的方面，但是本方面仍存在研究细致程度不足的情况。血糖变异性在糖尿病患者中的调控受重视程度日益提升，其在糖尿病并发症发生中的研究也可见[2-3]，但是其对糖尿病视网膜病变患者的影响研究极为匮乏。……
